Complex Care Clinical Lead Department: Clinical Reporting to: Head of Governance / Clinical Services Manager Base: Home and field-based Sentra Care is pleased to be partnering with a forward-thinking provider of high-quality community care to recruit an experienced and dynamic Clinical Lead. This is an exciting opportunity for a Registered Nurse with a strong background in community-based complex care, clinical oversight, and quality assurance. About This Clinical Lead Role The Clinical Lead is a highly experienced Registered Nurse responsible for providing expert clinical oversight to community-based complex care packages across multiple locations. This hybrid role involves travel to various service users for staff competency sign-offs, and quality assurance. The Clinical Lead plays a key role in the initial assessment and setup of new care packages, ensuring safe, responsive, and person-centred care aligned with regulatory and best practice standards. Responsibilities Clinical Oversight Maintain clinical oversight of high-risk or complex care packages, ensuring safe, compliant, and evidence-based delivery. Conduct regular reviews and competency checks on-site across all active locations. Identify and escalate emerging risks; contribute to lessons learned and audits. Assessment, Onboarding & Setup Participate in new care package assessments alongside the MDT. Support in ensuring care plans, risk assessments and staffing allocations are clinically appropriate from the outset. Review specialist clinical tasks where required. Crisis Management & Response Form part of the clinical response team for packages entering crisis. Provide urgent, short-term support to stabilise situations, complete safety reviews and lead staff debriefs. Liaise with senior leadership during escalation and ensure documentation is completed in line with safeguarding and governance processes. Workforce Competency & Supervision Travel to service user homes to complete hands-on staff training and sign-offs for clinical competencies (e.g., PEG, tracheostomy, suctioning). Provide clinical supervision and mentorship to RGNs and care staff, ensuring adherence to professional standards. Contribute to rota reviews and highlight clinical skill gaps to coordination or compliance teams. Multidisciplinary Collaboration Work in close partnership with the wider team including MDT clinicians, service leads, commissioners, and family carers. Represent clinical leadership in reviews and governance meetings. Quality & Documentation Ensure all clinical documentation is completed to a high standard and uploaded to Health Automated. Contribute to audits, policies and the embedding of clinical governance frameworks. Review incident reports and implement risk mitigation actions. Culture, Training & Staff Wellbeing Actively support the roll-out of staff wellbeing initiatives, reflecting the organisations values and commitment to a supported workforce. Lead by example in promoting a safe, learning-driven culture across the workforce.
