Crondall New Surgery is a welcoming, high-performing dispensing practice in a beautiful village between Farnham and Fleet. We are looking for a salaried GP (4 sessions) holding a patient list, to join our friendly, supportive team.

This is a role designed with sustainability in mind: manageable workload, strong continuity of care, and a team that genuinely looks out for each other. We are forward-thinking and involved in research and teaching, while using technology intelligently to reduce admin and improve workflow.

You will have the freedom to develop your own interests, with protected support for learning and development. Most importantly, you will be joining a practice that values work/life balance and a positive working culture as much as high-quality patient care.

Main duties of the job This is a 4-session role across 2 days, with a balanced mix of telephone and face-to-face consultations and responsibility for your own patient list, supporting continuity of care.

You will take part in a shared duty doctor rota (one late day per week until 6:30pm) and contribute to extended access on a light, rota basis, including occasional Saturday mornings and seasonal clinics.

We aim to keep workloads realistic and well-supported. You will work closely with a multidisciplinary team, with paramedic support for home visits, allowing you to focus on clinical care.

There is strong encouragement to develop specialist interests, with access to funding and excellent local educational opportunities.

About us Crondall New Surgery is a part-dispensing GMS practice serving 6,000 patients, with a steadily growing list. Located in a well-connected village on the SurreyHampshire border, we combine the benefits of rural practice with easy access to larger towns.

We are part of Fleet PCN (HIOW ICB) and have a well-established multidisciplinary team including GPs, nurses, HCAs, dispensers, a mental health practitioner, first contact physiotherapist, and an excellent admin team.

We prioritise continuity, personal care, and safe, efficient working. Our systems (EMIS Web, Ardens, Accurx) support streamlined workflows and reduce unnecessary admin.

We are known for being friendly, supportive, and collaborative, with regular meetings, shared lunches, and a strong emphasis on clinician wellbeing. Flexible working and portfolio careers are actively supported.

Job responsibilities The post holder will provide high-quality, patient-centred primary care services to the registered practice population. The Salaried GP will hold clinical responsibility for a registered patient list of up to 1500 patients, ensuring continuity of care and proactive health management.

The role includes delivering safe and effective clinical care, contributing to the achievement of contractual and quality standards, and working collaboratively within a multidisciplinary team to support the ongoing development of the practice and wider primary care network.

Clinical Responsibilities The Salaried GP will:

Deliver a high standard of clinical care to patients, managing both acute and chronic conditions in line with evidence-based practice.

Undertake comprehensive consultations, including history-taking, examination, diagnosis, treatment, and evaluation of patient needs from both physiological and psychological perspectives.

Formulate and implement appropriate, patient-centred management plans, ensuring shared decision-making.

Provide continuity of care for a list of 1500 registered patients, taking responsibility for their ongoing clinical management.

Proactively monitor and manage the health needs of the patient list, including recall, screening, and preventative care interventions.

Identify and prioritise patients requiring proactive or anticipatory care, including those with complex, frail, or long-term conditions.

Support and develop personalised care plans for patients, particularly those with multiple comorbidities or at risk of hospital admission.

Refer patients appropriately to secondary care and other services in a timely manner, in accordance with local and national guidelines.

Provide care in urgent and emergency situations, including initiation of appropriate emergency treatment.

Prioritise clinical workload effectively, ensuring safe management of complex and time-sensitive cases.

Manage long-term conditions proactively, in line with national frameworks, QOF requirements, and best practice guidance.

Promote health education and support patients in adopting healthy lifestyle choices and self-management strategies.

Prescribing and Medicines Management The post holder will:

Prescribe medications safely, effectively, and cost-efficiently, adhering to local formularies and national guidance.

Understand or learn the specific nuances, challenges and opportunities of dispensing practice.

Undertake regular medication reviews for patients on their list, in accordance with NICE guidance and local care pathways.

Monitor prescribing patterns and contribute to safe and effective medicines optimisation within the practice.

Administrative Responsibilities The Salaried GP will:

Complete all administrative tasks in a timely and accurate manner, including referrals, repeat prescriptions, test results, and clinical correspondence.

Maintain clear, accurate, and contemporaneous clinical records in accordance with GMC standards and practice policies.

Ensure appropriate follow-up, recall, and safety-netting arrangements are in place, particularly for patients within their defined list.

Manage incoming clinical information and take appropriate action to ensure continuity and patient safety.

List Holder Responsibilities The post holder will:

Hold responsibility for a defined patient list of up to 1500 patients.

Ensure continuity of care by maintaining oversight of patients ongoing medical needs.

Proactively manage population health within the list, including preventative care, screening uptake, and chronic disease management.

Monitor and improve quality outcomes for the patient cohort, including achievement of QOF, IIF and local enhanced service targets, applying the relevant coding to ensure achievement.

Identify high-risk or vulnerable patients and ensure appropriate care planning and multidisciplinary input.

Work with the wider practice team to ensure equitable workload distribution while maintaining list-based accountability.

Contribute to reducing health inequalities within the patient population.

Teamworking and Practice Contribution The post holder will:

Work collaboratively with all members of the practice team, sharing workload equitably.

Contribute actively to practice meetings and multidisciplinary team discussions.

Support the ongoing development of the practice, including participation in clinical governance, audit, and service improvement initiatives.

Take responsibility for delegated clinical or administrative areas, including Quality and Outcomes Framework (QOF) domains where appropriate.

Foster a positive team environment, promoting open communication and continuous improvement.

Work effectively with external organisations, including community services and the wider primary care network.

Governance, Quality, and Compliance The Salaried GP will:

Adhere to all relevant clinical guidelines, policies, and procedures.

Participate in clinical audit and quality improvement activities to enhance patient care.

Maintain up-to-date knowledge of current legislation and professional requirements.

Fulfil safeguarding responsibilities for both adults and children in accordance with statutory guidance.

Ensure compliance with infection control, health and safety, and information governance standards.

Contribute to the successful implementation of continuous improvement and quality initiatives within the practice.

Professional Development The post holder will:

Participate in the annual appraisal process and maintain a portfolio to support revalidation.

Engage in continuing professional development (CPD) and lifelong learning.

Contribute to the education, supervision, and training of colleagues and members of the multidisciplinary team.

Promote a culture of learning and professional development within the practice.

Flexibility The Salaried GP will:

Undertake additional duties as required, commensurate with the role, to support the effective functioning of the practice.

Respond flexibly to changing service demands, staffing levels, and organisational priorities.

Wider Responsibilities In addition to the core duties, the Salaried GP will:

Participate fully in clinical governance processes.

Attend and contribute to training events to promote best practice.

Provide clinical leadership, education, and mentorship to members of the multidisciplinary team.

Engage with local initiatives aimed at improving service delivery and patient outcomes.

Support collaboration across the primary care network and wider healthcare system.

Person Specification Experience

Experience in a primary care setting

Qualifications

Qualified General Practitioner with full GMC registration

Inclusion on the GP Register

MRCGP (or equivalent)

Evidence of ongoing CPD

Performers List registration

Evidence of current validation

Evidence of last appraisal

Eligibility to practice in the UK independently

Satisfactory Enhanced Disclosure and Barring Service (DBS) check, including barred list check, in line with NHS Employment Check Standards

The post holder will be expected to maintain strict confidentiality at all times and comply with all relevant data protection, health and safety, and practice policies.

Additional postgraduate qualification relevant to primary care

Qualification or experience in teaching/mentorship

Obs/Gynae experience/FRSH qualifications/HRT would be beneficial

An appreciation of the NHS landscape and relationship between individual organisations, PCNs and the commissioners
