Job Summary
We are looking for a pro-active, dedicated and flexible Community Psychiatric Nurse, to be responsible for the delivery of mental health nursing services, working as part of the multidisciplinary team who provide care within their scope of practice to the entitled patient population.

Main duties of the job
The Community Psychiatric Nurse CPN will be required to support the mental health services across our PCN, working within the Complex Care Team to address the needs of high intensity users in primary care.
The CPNs role requires the post holder to be able to work with, and understand the roles of, a variety of different people working in the practices and across the PCN in primary care, secondary care and community settings, e.g. doctors, nurses, mental health practitioners, healthcare assistants, social prescribing link workers, occupational therapists, physiotherapists, therapists and clinical pharmacists.
The role will be an integral part of the Complex Care Team.
About Us
Lincoln Healthcare Partnership (LHP) PCN is made up of two practices based in central Lincoln with a patient population of just under 40,000 people. The population of the LHP PCN has a higher percentage of working age adults and includes a high number of Lincoln University students as we run a practice on the University campus site. We have a smaller than average number of older residents and we also manage a number of care homes based in central Lincoln.
Primary care networks (PCNs) form a key building block of the NHS bringing general practices together to work at scale. Building on existing primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care for people close to home.
Being a city centre PCN with a younger than average population along with a higher level of deprivation and prevalence of mental health than the Lincolnshire average which influences our health care needs and the associated service provision.
The development of the network has allowed for a multi-disciplined team who work across our member practices enabling better delivery of care across the area.

The Following Are The Core Responsibilities Of The CPN Within Our Team. There May Be On Occasion a Requirement To Carry Out Other Tasks; This Will Be Dependent On Factors Such As Workload And Staffing Levels:
Working closely with the Complex Care Lead Practitioner, the CPN will:
Discharge professional responsibilities under the Mental Health Act (1983) and revised subsequent act Mental Capacity Act (2005) and the NMC Professional Code of Conduct
Be accountable and responsible for a defined caseload that reflects the priorities of the local population and carry responsibility for the assessment of care needs; under the supervision of the Complex Care Team Lead.
To develop, implement and evaluate evidence-based programmes of care; set standards of care and liaise with other agencies.
Participate in the multi-disciplinary assessment of individuals with mental health problems living in the community, by providing a community mental health nursing perspective and expertise
Ensure that a risk assessment is completed for service users, especially if there is a history of, or a potential for risk.
To adhere to professional confidentiality standards.
To be aware of the potential for vulnerability to abuse amongst the service users/families, and use of the local policy to inform appropriate personnel in line with safeguarding vulnerable adults and children.
Ensure that a carers assessment is offered to carers of service users, as in line with Standard 6 of the National Service Framework.
Help develop and maintain effective joint working arrangements with colleagues in the community mental health services, residential services, primary health care, social services and voluntary services, thus facilitating the co-ordination and comprehensiveness of provision.
Utilise the full range of mental health nursing skills to participate in the assessment, care and support of service users.
Use specialised knowledge, experience and skills in direct clinical work and act as a resource for the team and other agencies. This may include work in rehabilitation, social interventions, development of social networks and accessing other services/resources, mental health promotion, counselling and intercultural therapy, always encouraging service user participation.
Participate in the provision of community mental health nursing advice andconsultation to colleagues, community groups and organisations.
To act in a manner to respect the customs, individuality, values, sexuality and spiritual beliefs, actively supporting the individual to fulfil these.
Ensure detailed and appropriate records are maintained to demonstrate that contact with people who use services, and their carers, is meaningful and effective.
