Role Overview
1. Crucial role in supporting vulnerable adults in the community

Key Responsibilities
5. Assessments and Support Planning
6. Conduct comprehensive assessments considering various factors.
7. Coordinate services with health professionals and community resources
Safeguarding and Risk Management
8. Promptly identify and respond to safeguarding concerns.
9. Assess risks and implement interventions.
10. Ensure safety and legal compliance.
Direct Work with Clients
11. Provide emotional support, advocacy, and access to services.
12. Facilitate group sessions to enhance social skills.
Collaboration and Multi-Agency Working
13. Liaise with healthcare professionals, police, etc.
14. Advocate for clients’ rights in multidisciplinary meetings.
Documentation and Record Keeping
15. Maintain accurate case records and ensure compliance.
Qualifications and Experience
16. Degree in Social Work or equivalent.
17. Registration with Social Work England.
18. Experience in adult social care.
19. Knowledge of relevant legislation.
