As a community nurse, you would be supported to develop into an autonomous practitioner; nursing patients with multiple complex health care needs in their own home.
Main duties of the job
1. To contribute to processes for service and quality improvement
2. Assess the physical and psycho–social needs of patients and deliver therapeutic interventions based on best available evidence in order to improve health outcomes.
3. To manage own case load, delivering and evaluating service delivery to patients with a variety of medical conditions and those delegated by case managers.
4. To plan interventions with the Case Manager and other team members using the common assessment process
5. To take part in the referral management process to ensure patients are treated by the most appropriate member of the team/service
6. To contribute to the development of protocols and procedures developed for clinical pathways
7. Assist in the development of case management protocols and guidelines within their own area of professional practice
8. Participate in weekend rotas / 7 day working rotas

EDUCATION, QUALIFICATIONS & TRAINING
Essential criteria
19. Registered Nurse or student nurse awaiting confirmation of NMC registration
Desirable criteria
20. Post registration education and extended skills eg Intravenous drug administration, compression dressings Desirable but not essential
21. Post registration education and extended skills e.g. Intravenous drug administration, compression dressings Desirable but not essential
SKILLS, ABILITIES & KNOWLEDGE
Essential criteria
22. To be able to communicate effectively with patients, relatives, carers and other professionals.
23. To be able to care for patients without prejudice related to gender, religion or disability.
24. Able to work under pressure
25. Ability to manage own Caseload
26. Knowledge of the NMC Code of Professional Conduct
27. Knowledge of the Data Protection Act
EXPERIENCE
Essential criteria
28. Demonstrate experience post or pre-registration placements in relevant clinical areas: Care of Elderly Community Orthopaedics Neurology Respiratory Palliative Care Long term conditions
Desirable criteria
29. Knowledge of working with a single assessment, planning and the delivery of care
30. Experience of electronic clinical systems and technology e.g. RIO
SPECIAL CIRCUMSTANCES

31. Car driver / owner able to travel to other locations within the trust
