NorthYorkshire Council are recruiting for a full time Social Worker to join theirteam in the Selby Area. The role offers the flexibility of hybrid working froman agreed office base.
Reporting to North Yorkshire Council, Health and Adult Services, Intermediate Care Managers and Collaborative Care Team you will be part of a small, supportive dedicated team working in Urgent Care. You will work closely with our social workers, social care coordinators, therapists, nurses and healthcare support workers towards preventing admission to hospital and facilitating early discharge to a range of care settings.

Main duties of the job
You will berequired to work at pace, supporting short-term interventions with people,their families and carers. This may involve undertaking a range of assessmentsand developing personalised care and support plans that enable people who areunwell or in a health crisis to remain in their own homes.
You willdetermine service eligibility for adults with care and support needs againstthe national standards and ensure that patients and carers are safe from harm.Where harm or abuse has been identified, you will support the team to progressSafeguarding work.

Job context
Social Care Coordinators (SCCs) work with adults with careand support needs and carers in the community and may be part of the CommunitySocial Care Team, Discharge Hub or Review Team.
This part of the pathway manages and provides services forthose adult with care and support needs and carers that require furtherintervention or specialist interventions for example sensory services.
Community Social Care teams describes the part of theservice aimed at supporting adults with care and support needs and carers withmedium term goals and longer term outcomes again embedded in the values andprinciples of recovery and maximising independence. The service will provide a care co-ordinationfunction ensuring care is co-ordinated around the adult with care and supportneeds and carers and every opportunity to maximise independence and promotewellbeing are taken advantage of.Effective support planning and asset and strength based approaches toassessment and review are essential elements of this service. Community Social Care will be a generic adultsocial care team and will provide services to adults with care and supportneeds and carers, these may include people with a learning disability, autism,older people and people with physical disabilities. Sensory services willmaintain a specific specialism within Community Social Care.
The Locality Discharge Hub Teams are linked to key AcuteHospital sites. The primary focus of this team is to support timely and safehospital discharges for people to the community.
Services in the Discharge Hub and Community Social CareTeams will operate over 7 days. Therefore, you will be required to work on aweekend, on a rota basis. The specific requirements will be determined byservice and operational team requirements.
The Review team operate on a countywide basis. The mainpurpose of the team is undertake the Review duties of the Local Authority forpeople in long term residential care settings.This team operates with a mix of Social Workers and Social CareCo-ordinators who will undertake a blend of face to face and remote working.Working closely with the Quality Improvement and Quality Monitoring teams, thisteam will reduce the risk of safeguarding concerns, and will complete allreassessments for people where there are organisational concerns within theresidential care settings. Where necessary commission additional support or newprovision to meet identified outcome and needs.
Job Purpose:
To undertake a range of asset and strength based assessmentsand interventions to ensure that adults with care and support needs and carersachieve their desired outcomes and wellbeing.To determine eligibility for adult social care service set against thenational standard. To work with adults with care and support needs and carersto develop personalised care and support plans that represent best value. To ensure that adults with care and supportneeds and carers are safe from harm.
Operational management:
Undertake timely assessments of need with adults with careand support needs and carers to determine a persons level of need. Allassessments and reviews are undertaken in line with relevant legislation andbest practice e.g., asset and recovery based, personalisation and personcentered approaches.
Provide support and information, advice, and guidance toadults with care and support needs and carers on how their needs could bepartly or wholly met by access to universal and other non-care services,including preventative services.
Support the uptake of direct payments to meet the adults orcarers outcomes.
Or commission, with others, services to meet the adult's orcarer's needs, including working closely with Brokerage colleagues.
Ensure value for money and maximise opportunities togenerate income for adults with care and support needs and carers.
Assess for and arrange disability equipment and adaptationsto maintain / improve the adult with care and support needs or the carer'sindependence in daily living activities.
Fit simple equipment, demonstrate to, and educate adultswith care and support needs and carers and appropriate colleagues/care workersin the correct use of equipment and/or techniques to enable the adult toachieve optimal independence in activities of daily living. Including referringindividuals to the Reablement team and providing support to this team.
Undertake re-assessments and reviews of support packagesusing an asset and strengths-based approach.
Resource management:
Following assessments, identify whether or not the adultand/ or carers fall within the national eligibility criteria, and communicatethis to the person.
Promote and support adults with care and support needs andcarers to access and use direct payments, other self-managed budgets,self-directed support and self-assessment.
Where the adult with care and support needs or carer fallwithin eligibility
criteria, to work creatively and innovatively with the adultand / or carers to develop an agreed support package to meet the identifiedneed whilst maximising resources and within the assessed indicative budget.
Support the adult with care and support needs and carers tomeet their assessed needs within the indicative budget and in a way whichmaximises resources, where this is not the case you will seek advice from yourline manager at the earliest opportunity.
Maximise an adults ability to manage their own resourcesand circumstances, enabling them to maintain their independence for as long aspossible, including managing risk in a positive manner, avoiding paternalising/ maternalising people.
Explore opportunities to meet desired outcomes throughalternative funding streams e.g., voluntary sector or Continuing Health Carefunding
Partnerships:
Work with colleagues within Heath and Adult Services, Careand Support, North Yorkshire Council, also other agencies including healthpartners and voluntary sector to ensure you maximise resources andopportunities for people.
Involve advocacy services when appropriate
Communications:
Maintain clear, concise, and timely records of cases, carepackages and actions in line with the Directorate's policies on filemaintenance, this includes electronic and paper records - the primary method ofrecord keeping is electronic.
Communicate with adults with care and support needs andtheir carers in an appropriate manner to enable effective written and oralcommunication and in line with Directorate policies and procedures.
Effectively negotiate and manage conversations where thereis a disagreement, with adults with care and support needs, carers and others
Engage with a range of other agencies to maximise choice andresources.
Communicate effectively and in a manner and timescaleappropriate for the level of urgency, to your team manager, other colleagues,and professionals.
Maintain regular contact with adults and carers to monitorand respond to changes in their situations.
Systems and information:
Utilise the current business processes in relation to recordkeeping, financial monitoring and ICT.
Assist in the collection of data using the appropriate ITsystems, including keeping up to date with and supporting organisationaltechnical developments.
Safeguarding:
Operate as a Safeguarding Adults Alerter and also berequired to intervene in emergency situations to protect vulnerable adults andto initiate the appropriate statutory and other actions required. Postholdersare also expected to assist with Safeguarding Adults Investigations, asdelegated by the Designated Safeguarding Manager and in line with procedures.
To be committed to safeguarding and promote the welfare ofchildren, young people, and adults, raising concerns as appropriate.
Person Specification
Experience
* Knowledge of Equality & Diversity, Dignity & Respect and Human Rights legislation.
* Knowledge of the basic framework of the Care Act and its application.
* Knowledge of the Mental Capacity Act.
* Knowledge of Direct Payments.
* Knowledge of basic health and social care policy developments and good practice.
* Understanding and application of the Social Model of Disability.
* Knowledge of the Data Protection Act and confidentiality.
* Experience in an adults Health or Social Care setting, working with people with physical or sensory impairments or with older people.
* Demonstrable experience of assessing adults needs in either a community, hospital or residential setting.

Qualifications
* Either hold a Health and Social Care NVQ Level 3 diploma (Adults) or be willing to obtain this qualification following appointment to the role
* Equivalent vocational qualifications are Health & Social Care NVQ Level 3 (adults route), NVQ 3 in Promoting Independence, CQF Level 3 Diploma in Health & Social Care
* NVQ 4 in Care.
* The following professional qualifications would be accepted in place of the Health and Social Care NVQ Level 3 (adults route) or CQF Level 3 Diploma in Health & Social Care: Social Worker, Registered Nurse, and Occupational Therapist.
* If professionally qualified to have a registration with the relevant professional body.
