Position Overview: We are looking for a dedicated Care Team Leader to oversee and coordinate the delivery of care services to thier clients. The Care Team Leader will be responsible for leading a team of caregivers, ensuring the highest standards of care are maintained, and fostering a supportive and collaborative work environment.
Key Responsibilities:
* Supervise and mentor a team of caregivers, providing guidance, support, and ongoing training as needed.
* Coordinate client care schedules and assignments, ensuring appropriate staffing levels to meet client needs.
* Conduct regular assessments of client needs and develop personalized care plans in collaboration with clients, families, and healthcare professionals.
* Monitor the quality of care delivered by the team, conducting regular evaluations and implementing improvement strategies as necessary.
* Act as a liaison between clients, families, caregivers, and management, addressing any concerns or issues promptly and effectively.
* Ensure compliance with company policies, procedures, and regulatory requirements related to care delivery and documentation.
* Participate in the recruitment and onboarding of new caregivers, conducting interviews and assisting with orientation and training programs.
Qualifications:
* Proven experience in a caregiving role, with a minimum of [2] years of experience in a supervisory or leadership position.
* Strong interpersonal and communication skills, with the ability to effectively collaborate with diverse stakeholders.
* Excellent organizational and time management abilities, with the capacity to prioritize tasks and manage multiple responsibilities simultaneously.

* Proficiency in using computer software and electronic health records systems.
* Certification in CPR and First Aid (preferred).
