Are you an exceptional clinical pharmacist ready for a new challenge? Our pharmacy team needs capable and enthusiastic individuals with the right values to be a key part of our dynamic teams in areas such as Hepatology (including liver transplant), Renal Medicine/Transplantation, Surgical specialties (including Acute Surgery, Upper GI/Pancreas), Surgical Oncology, and Urology. The posts comprise a continuous, one-year clinical rotational programme designed to guide you through the Advanced Level Framework.
You will have clinical pharmacy experience and a diploma. If you are not already a prescriber, we support you in gaining this qualification.
Post holders will provide medicines optimisation services to patients while integrating into the multidisciplinary team. We also support the Abdominal Medicine & Surgery CSU by providing clinical pharmacy services to Gastroenterology and Colorectal Surgery. Additionally, experience in anticoagulation clinics (including warfarin bridging) and Aseptic dispensing in our busy Aseptic & Production Unit is offered.
You will receive support from our advanced clinical pharmacists and appropriate training. You will be encouraged to offer ideas, collaborate to improve patient care and experience, and play a key role in service development implementation.
Main duties of the job
The team works closely with specialty teams and attends ward rounds to impact patient care through MDT collaboration.
Pharmacy Services at LTHT are nationally recognized as an exemplar. The CSU responsibilities include the provision and use of medicines across all patient pathways.
You will be supported by our advanced clinical pharmacists and receive relevant training. Your role includes supervising rotational junior pharmacists and supporting pharmacy technicians in their emerging clinical roles.

Job responsibilities
JOB PURPOSE
To provide pharmaceutical care to a group of patients within a clinical area, rotating specialties every 12 months. To support the Lead Pharmacist by assisting in policy and procedure development, financial reporting, and horizon scanning activities.
JOB DIMENSIONS
Provide specialist clinical pharmacy services within each rotation, being professionally accountable for actions, and offering drug-related advice to patients and healthcare professionals. Monitor drug use, advise on therapy optimization, report near misses, and participate in education and training. Support operational services including dispensaries, Medicines Information, and aseptic services. The role requires working evenings, weekends, bank holidays, and participating in an on-call rota.
