Job summary
TheNurse Practitioner / Clinician is responsible for managing and providing carefor the presenting patient with undifferentiated undiagnosed conditions, frominitial history taking, clinical assessment, diagnosis, treatment, prescribingand evaluation of care. To include acute, minor and long term conditions inline with existing practice and national policies and guidelines.
Youwill work within your own professional competence in accordance with the NMCCode of Conduct. The team member will provide a service which is flexible,sensitive and responsive to the health needs of those living and working in thearea, in particular ensuring the ethnic, linguistic and cultural needs aretaken into account.
Main duties of the job
1. To provide direct access to all patients, adults and children seeking primary health care. Both face to face, home visits and on the telephone. To be involved with On Call sessions and triage.
2. The post holder is required to work within the policies and procedures of the practice and with due regard to the NMC Code
3. Assess diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis, referring to secondary or community care as appropriate
4. Assess diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs, to maintain extensive knowledge of chronic disease management and their processes and management
5. Proactively identify, diagnose and manage treatment for patients at risk of developing long term conditions
6. Prescribe and review medication for therapeutic effectiveness appropriate to the patients needs and in accordance with national and practice protocols and within scope of practice
7. Work with patients and pharmacists to support concordance to treatment and provide information and advice on prescribed or OTC medication- side effects and interactions
8. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations
9. To prioritise and develop the managementof housebound patients managing complex needs and multipathology, liaising withGPs, voluntary sector, district nurses or community matron as appropriate

19. Support patients to adopt health promotion strategies and self-care, referring as appropriate
20. Support and manage health needs of women presenting for family planning, cervical cytology or sexual health consultation
21. To prioritise and develop the management of housebound patients managing complex needs and multipathology, liaising with GPs, voluntary sector, district nurses or community matron as appropriate
22. Work closely with our GPs in audit and analysis of audit
23. Developing models of good practice from internal and external sources to improve clinical care within the practice team and hence help in reducing admission, prescribing and duplication of work.

24. Job plan that demonstrates advanced nursing practice and has equity with peers working at this level
25. Experience of prescribing and undertaking medication reviews
26. Experience of working in a primary care environment
Qualifications
Essential
27. Registered Nurse with Nursing and Midwifery Council
28. Masters degree required for qualification post December 2020 refer to RCN Credentialing for Advanced Level of Nursing Practice.
29. Post graduate diploma or degree for Advanced Practice Qualification up to December 2020
30. Qualified Independent Nurse Prescriber on the NMC register
31. Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet
32. Minor illness qualification
