We are recruiting a Home Support Scheduler to join our Home Assessment Team – Out of Hours in North Lanarkshire. This role focuses on delivering timely, person‑centred support, facilitating same‑day discharge for hospital patients returning home.
Key Responsibilities
• Use a dynamic scheduling tool to ensure prompt, efficient support for service users, promoting greater independence and choice.
• Work closely with Health & Social Care and Acute colleagues to prevent delays in hospital discharge arrangements and support families during the first days at home.
• Communicate effectively with service users, families and partner agencies.
Qualifications & Experience
* Previous experience in a social work or home support related environment is essential.
* Excellent organisational skills, resourceful and able to work well under pressure.
* Positive approach, excellent communication skills with a range of individuals.
* Possession of a SQV Level 3 or equivalent qualification is desirable; applicants without this qualification must demonstrate a willingness to work towards achieving it within a reasonable timescale.
Work Pattern
Week 1: Monday, Tuesday, Friday, Saturday, Sunday – 12:30 – 22:00
Week 2: Tuesday, Wednesday, Thursday – 12:30 – 22:00
Week 3: Monday, Tuesday, Friday – 12:30 – 22:00 & Saturday Sunday – 07:00 – 16:30
Week 4 – Tuesday, Wednesday, Thursday – 12:30 – 22:00

*For all casual/sessional roles, you will receive 12.1% of your hourly rate for every hour worked in total recompense for annual leave, including public holidays. These rates will be indicated separately on your payslip. You will only be paid for the hours that you work.
