Shift work 4 days on 4 off split shifts between the hours of 7.00am - 10.00pm over an 8 week rolling shift pattern. Part time hours may be considered within these working hours / shift patterns.

The Reablement and Home First Team provide vital short-term services to people by helping them regain or attain as much independence as they can in their own homes. The therapy-led team works to resettle people after a hospital stay and to identify and work on goals that will help them live well at home.
Main duties
Senior Rehabilitation Assistants work with our Occupational Therapists, Physiotherapists and Reablement Coordinators to undertake assessments and provide reviews to ensure people are receiving the right level of input and/or are working on their outcome-based goals.
Senior Rehabilitation Assistants work as part of a close team undertaking activities according to people's individual goal plans, encouraging them to do as much as possible for themselves, to live well and safely in their own homes. They also work with the Reablement coordinators to support and maintain quality standards of work delivered by the Rehabilitation Assistants.
Your role will involve completing assessments, determining risks and how to manage them, communicating and recording your feedback to others involved in people\'s rehabilitation.
You will need to hold a valid driving licence and have access to a vehicle to enable you to drive around the locality so you can work with people in their homes. Team working and communication skills are essential. You will be confident with handling IT systems to update written notes and electronic records.

QUALIFICATIONS
* Essential: NVQ in a relevant topic (such as health care) or other relevant training
* Desirable: A level pass (or equivalent)
LENGTH AND / OR NATURE OF EXPERIENCE
* Essential: Experience of working in a health or social care environment
* Experience of working with people, their families and carers who may be going through challenging experiences or difficult situations
* Experience of utilising professional judgement to undertake assessments/review progress in a health or care setting
* Practical experience of motivating service users; encouraging independence
* Desirable: Demonstrated experience of goal setting with people/patients
