Nocturnal Recruitment Solutions are looking for a Young People Support Worker support worker who can work with young people in a supported living service. The work is based within North/North West London, and the ideal worker will have an extensive background dealing with vulnerable 16-21 year olds who have diverse needs such as challenging behaviour, mental health and emotional needs. As a Young People Support Worker you will need to Have a Level 3 Diploma in Health and Social Care or Residential Childcare or Relevant Bachelors Degree (or Higher) e.g Social Work, Youth Studies. Roles and responsibilities: * Assess practical and emotional needs, developing care plans that outline the required care and support in a person-centred approach. * Ensure risk assessments are actioned as necessary, in line with policies and procedures. * Be responsible for the delivery of services to children and young people in the home, in such a way that safeguards and promotes their welfare. * Collate data statistics and undertake administrative duties as well as completing daily logs. * Work in partnership with other professionals to ensure that the individual gets the support they need. * Accompany residents in meetings with other agencies and health professionals as well as liaising with other support workers, social workers, YOs * Work with residents’ families to help them to support the young person in the most effective possible wa...
