Make a tangible impact on patient care at the frontline of medicines optimisation. As a Clinical Pharmacist, you will be an integral member of the multidisciplinary ward team, delivering a high-quality, patient‑centred clinical pharmacy service to inpatients. Working collaboratively with ward clinicians, practice pharmacy teams, GPs, and carers, you will play a key role in ensuring the safe, effective, and cost-effective use of medicines.
You will apply your clinical expertise to support evidence-based prescribing decisions, optimise treatment outcomes, and enhance medicines safety. In addition, you will contribute to seamless patient care across the wider healthcare pathway, supporting continuity and improving overall patient experience. This role is based at Swindon Intermediate Care Centre, a modern healthcare facility providing high‑quality intermediate and inpatient care. The post is contracted at 18.75 hours per week, offering a flexible opportunity to deliver meaningful clinical impact within a supportive care environment.
