Job summary
A fantastic opportunity has arisen and North Preston Medical Practice are looking to recruit an Advanced Clinical Practitioner (ACP) to complement our team. We are looking for an ACP to consult autonomously within their professional scope of practice providing face to face and telephone consultations. The ACP will be enthusiastic, flexible, reliable with Independent Prescribing status.
The ACP will work within their sphere of competence and will provide care for our patients. You will demonstrate safe, competent clinical decision making. You will work collaboratively with the practice team to meet the needs of our patients.
The role will be full time hours with scope to work part time at 30 hours for the right candidate. We have an excellent clinical and administrative team to assist in the delivery of services to patients.

The following are the core responsibilities of the Advanced Clinical Practitioner.

9. Practice in compliance with their respective code of professional conduct and within their scope of practice, being responsible and accountable for their decisions, actions and omissions at this level of practice
10. Demonstrate a critical understanding of their broadened level of responsibility and autonomy and the limits of own competence and professional scope of practice, including when working with complexity, risk, uncertainty and incomplete information

23. Critically assess and address own learning needs, negotiating a personal development plan that reflects the breadth of ongoing professional development across the four pillars of advanced clinical practice (clinical practice, leadership and management, education and research).
24. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
25. Maintain accurate clinical records in conjunction with extant legislation and to ensure SNOMED CT codes are used effectively
26. Critically appraise and synthesise the outcome of relevant research, evaluation and audit, using the results to underpin own practice and to inform that of others
27. Develop and implement robust governance systems and contribute to practice targets (QOF, etc.), complying with local and regional guidance
28. Chaperone patients where necessary
29. Support the team in dealing with clinical emergencies
30. Support the team with all safeguarding matters in accordance with local and national policies
31. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
32. Deliver opportunistic health promotion where appropriate
33. Conduct comprehensive patient assessments, including medical history, physical examinations, and diagnostic tests
