We are supporting a client in Sefton in recruiting a dedicated Leaving Care Personal Advisor to work with young people aged 16-25 who are preparing to leave, or have recently left, care. You will play a key role in promoting independence, improving outcomes, and ensuring each young person receives the support, guidance, and opportunities they need to thrive. Key Responsibilities Provide advice, guidance, and practical support to care-experienced young people. Take part in the assessment, preparation, and review of Pathway Plans with a person-centred approach. Coordinate services and ensure young people access the support available to them. Maintain regular contact with young people and monitor their progress and wellbeing. Promote positive health, wellbeing, education, and independence skills. Identify risk, safeguarding concerns, and escalate appropriately. Work collaboratively with internal teams and multi-agency partners across Sefton. Attend relevant meetings, representing the service as needed. Maintain accurate and timely case records using ICS and other systems. Support service development by contributing to meetings, reviews, and training. Key Requirements Experience supporting young people, ideally those who are care-experienced or transitioning to independence. Strong understanding of safeguarding, risk management, and multi-agency working. Ability to build positive, professional relationships with young people aged 16-25. Good knowledge of Pathway Plans and legislation relating to care leavers (desirable). Effective communication, record-keeping, and organisational skills. Contract Length: 3-6 months with a view to extend to an ongoing contract.
