We are looking for a Rehabilitation Assistant to join the IntermediateCare team, which is part of Responsive Services team in First CommunityHealth and Care.
The Rehabilitation Assistants work collaboratively with themultidisciplinary team including Physiotherapists, Occupational Therapists,Nurses, other Rehabilitation Assistants and Administrative Staff. TheIntermediate Care team provides short term care packages and rehabilitationto patients who have recently been discharged from hospital or those at riskof being admitted to hospital. The post of Rehabilitation Assistant involvesproviding care visits and rehabilitation in the patients own home under thedirection or our nurses and therapists.
Due to nature of the role it is essential that applicants are able todrive and have access to a car suitable for business use. The use of publictransport to complete this role is not possible due to the area wecover.
The service operates from 08:00-20:00, 7 days a week, 365 days a year,the post holder will be expected to work on a rota basis.

* An interest in working in rehabilitation of patients in the home settingto improve patients independence and achieve their goals
* Experience of working autonomously and as part of a team
* Must be able to manage time effectively
* A willingness to undertake competencies to enhance skills in the role.

Job responsibilities
The post holder isresponsible for working under the direction of the nurses and therapists. You will work within the multidisciplinary team, promoting independence by providingsupport and care in the community setting.
The post holder will carry outdelegated tasks given by the Physiotherapist, Occupational Therapist and the Nursing team.
The service operates 8am to8pm, seven days a week.

Person Specification
Experience
* Previous nursing experience or experience of caring for someone who was elderly, disabled or ill, not necessarily in paid employment
Knowledge
* Good written communication skills and an ability to maintain accurate, clear records
Other
* Willingness to learn new skills to adapt positively to changes in working practice and patterns
* Able to work flexibly in order to provide personal care and support
* Access to car to drive for work purposes
Skills
* Good communication skills and interpersonal skills, including an ability to build rapport and establish good one to one relationships
* Ability to organise own time efficiently
* Ability to work effectively as part of a team and to maintain a positive team-working relationship by relationship by valuing the contributions of other team members, understands others needs, using flexible approach, handling conflict positively in an open and constructive manner

Qualifications
* Good general knowledge and education to GCSE standard.
* NVQ levels 1 & 2 or equivalent in health and social care
* NVQ 3 or studying towards CQF diploma level 3 in clinical health care support
