Rate of Pay: £17.85- £24.42 per hour
Summary:
The Senior Social Care Assessor plays a key role in delivering high-quality, person-centred Adult Social Care services within the Community Wellbeing service. Working as part of a multidisciplinary team, the postholder will undertake strengths-based assessments, care management, reviews, and mental capacity assessments to support adults, young adults transitioning into adult services, informal carers, and vulnerable individuals to maintain independence, wellbeing, and positive outcomes.
Responsibilities:
* Undertake strengths-based, person-centred assessments of adults, young adults, and carers to identify needs, risks, outcomes, and support requirements.
* Support adults and carers to identify personal outcomes and develop strengths-based plans that promote independence, resilience, and wellbeing.
* Identify safeguarding concerns, assess risk, and take appropriate action in accordance with safeguarding procedures and adult protection policies.
* Ensure compliance with Adult Social Care legislation, council policies, professional standards, and statutory responsibilities.
* Participate in supervision, professional development, training, and reflective practice activities to maintain effective professional performance.
Qualifications:
* NVQ Level 3 or above in Health & Social Care, Social Care, Community Care, or a related discipline, or equivalent relevant qualification and/or demonstrable experience.
* GCSEs (or equivalent) in English and Mathematics.
* Evidence of continuous professional development relevant to Adult Social Care practice.
Essentials:
* Good understanding of Adult Social Care legislation, statutory guidance, and professional practice frameworks.
* Understanding of relevant legislation including the Care Act 2014, Mental Capacity Act 2005, safeguarding responsibilities, and adult wellbeing principles.
* Experience working within a care, health, or Adult Social Care environment.
* Experience supporting adults, carers, or vulnerable individuals to identify goals and outcomes and develop strengths-based plans.
