Overview
An exciting opportunity has arisen for an enthusiastic, motivated and conscientious individual to join our Community Specialist Diabetes Team as an Advanced Clinical Practitioner. The service is currently delivered between the hours of 9am-5pm, Monday to Friday, across the Sefton geographical area. The post holder will support the Community Specialist Diabetes team with mentorship, clinical supervision and development of others, and will help ensure a consistent, quality-driven delivery of service. The post holder is required to deliver an advanced level of clinical practice within their recognised remit.
Responsibilities
* Deliver an advanced level of clinical practice within their recognised remit.
* Exercise independent judgment to assess, investigate, diagnose, plan, implement and evaluate the clinical care and management of patients within their care, within local and nationally agreed frameworks.
* Provide the team with advanced clinical leadership and undergo training to meet all academic and core clinical requirements (the four pillars of advanced practice) relevant to the role.
* Support the development of others through mentorship and clinical supervision and contribute to establishing systems and processes aligned with the trust strategy.
* Work within the Sefton Place Leadership Structure and develop key working relationships with nursing, medical and multidisciplinary teams, trust management teams and wider partners.
