We’re looking to recruit an experienced and enthusiastic Band 6 Senior Community Learning Disability Nurse with a focus on health facilitation.

Main duties of the job

* To participate in the delivery of a countywide specialist Learning Disability nursing with a focus on reducing health inequalities, under the guidance of the lead Nurse.
* To provide professional supervision to other staff /students as appropriate including appraisal and professional development particularly mandatory training.
* To contribute to the planning, development, and evaluation of learning disability services, including contributing to defined projects as delegated by the LDP management team via the Lead Nurse.
* As an autonomous practitioner to manage a specialist case load providing assessment, identification and treatment of the health needs of adults with a learning disability.
* To participate in multi disciplinary/multi agency working within the LDP and with Key partners.
* The role will have a focus on health facilitation, health promotion and linking with primary, secondary health and neighbourhood teams to reduce health inequalities.

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You need to hold a full UK driving licence and have use of a vehicle. Public transport is not acceptable for this role. Please confirm in your application that you meet the specified criteria.


Detailed job description and main responsibilities

1. The post holder will demonstrate knowledge and understanding of working with adults with a learning disability and be willing to undertake appropriate post registration training.
2. To understand health and social care policy and legislation for the service user group in relation to professional practice.
3. To keep up to date and support the implementation of national recommendations including LeDeR findings and other health initiatives, to support reducing health inequalities and improve the health of people with a learning disability.
4. To have a good understanding of the specific needs of the service user group including at times of crisis with a physical health, screening and health facilitation focus.
5. To recognise the impact of a person’s communication needs and tailor interaction to meet this need.
6. To have the ability to communicate very sensitive or distressing information to service users and carers as required.
7. To recognise the impact physical health has on people with a learning disabilities wellbeing, mental health and behaviour and work to reducing health inequalities.
8. To plan, implement and regularly review nursing care plans.
9. To work to reduce health inequalities for people with a learning disability by working with partners in primary, secondary care and neighbourhood teams including delivering training and education as required.
10. Support initiatives to improve the uptake of screening, vaccinations and annual health checks for people with a learning disability.


Person specification


Education/Qualifications

* RNLD or equivalent LD Registration with Nursing and Midwifery Council
* Teaching and assessing in clinical practice or mentorship
* Additional clinical training relevant to specialism


Experience

* Experience of working with adults with learning disability
* Experience of managing a complex case load
* Experience of completing nursing assessments and interventions relevant to adults with learning disabilities
* Post qualifying experience within a community MDTExperience of working across organisational and inter-agency boundaries
* Experience in providing clinical supervision

Other Requirements

* Ability to travel independently around the Cambridgeshire and Peterborough area meeting required timescales. A full driving licence and access to a vehicle is essential.
