Job Overview
We are recruiting Community Health Care Support Workers to support our District Nursing Teams based in Milton Keynes. The role involves delivering high quality, patient‑centred care in the community, supporting registered nurses with clinical care and rehabilitation in patients’ homes.
Key Responsibilities
You will receive training and support to practice confidently and safely in the community. Core duties include:
* Wound care management, leg ulcer management and vascular assessments including compression bandaging
* Female and suprapubic catheterisation and basic life support
* Supporting the implementation of care plans, assessing patients, and contributing to outcomes
* Maintaining accurate, timely clinical records in both electronic and handwritten formats
* Communicating with the multi‑disciplinary team and using handover tools
* Adhering to the Code of Conduct for Health Care and Support Workers and Adult Social Care Workers in England
The service operates 7 days a week on a rota system (09:00hrs – 17:00hrs) with approximately one weekend per four. A full UK driver’s licence is essential, and petrol drive or reliable access to transport is required.
Person Specification
Qualifications
* Essential – Numeracy and literacy skills to keep accurate records and administer medicines where appropriate
* Essential – Spoken and written English to an appropriate standard
* Desirable – NVQ/L3 or equivalent
Experience
* Essential – Previous experience working with a multi‑disciplinary team
* Essential – Experience communicating with the general public and working in a customer‑focused, fast‑paced environment
* Essential – Experience in a caring role, and the ability to work independently under the supervision of nurses
* Desirable – Healthcare/NHS or similar experience in community or hospital settings
* Desirable – Experience in a mental health setting or working with adults with mental health problems
* Desirable – Volunteer sector experience
* Desirable – Experience in a caring role
Skills and Knowledge
* Essential – Strong communication skills with the multi‑disciplinary team, using handover effectively and speaking face‑to‑face and by telephone
* Essential – Maintains accurate, concise, legible clinical records, including electronic patient records and Microsoft Word reports
* Essential – Applies the Mental Capacity Act and Deprivation of Liberty Safeguards
* Essential – Works under own initiative while following care plans and Band 4 staff directions
* Essential – Contributes to care plans and supports patients’ rehabilitation goals
* Essential – Demonstrates compassion, respect, reliability and enthusiasm
* Desirable – Basic knowledge of the Mental Health Act 1983
