Overview

The Urgent Community Response (UCR) Service provides urgent support seven days a week to help prevent unnecessary hospital admissions. It delivers a two-hour crisis response by a multi-skilled team of professionals, at home or in the patient’s usual residence. The service collaborates with local ambulance services to help category 3 & 4 patients receive a rapid community response and maintain independence at home. A general virtual ward is also provided, supported by digital technology and remote monitoring, enabling hospital-level care at home when appropriate.

Teams include GPs, nurses, paramedics, pharmacists, occupational therapists, physiotherapists, and health and therapy assistants. Patients are reviewed daily by a clinical team, and ward rounds may be conducted at home or via video technology. The service operates 7 days a week, 365 days a year, between 8am and 8pm. The UCR Service offers clinical interventions as part of a multidisciplinary team for admission avoidance pathways in line with national specifications for UCR and virtual wards.


Responsibilities

* Provide clinical expertise and problem-solving support for the nursing team and deliver nursing care to patients requiring nursing interventions.
* Coordinate the multidisciplinary team and allocate visits as appropriate.
* Develop the service through reflective practice, clinical supervision, and audit.
* Identify suitable people referred to the service for planned early discharge from hospital or for prevention of admission to acute or long-term care.
* Work within the service to triage, assess, and deliver care to patients over several days to enable them to remain safely at home.


Qualifications and requirements

* Clinical experience and ability to coordinate a multidisciplinary team in an urgent community setting.
* Willingness to participate in reflective practice, supervision, and audit activities.
