Job responsibilities JOB PURPOSE: The Prevention Services Lead Nurse will provide visible nurse leadership, working as part of an agile team supporting Herefordshire General Practice, ensuring the safe and effective delivery of Prevention Services. You will be supporting priorities within the Herefordshire Health & Wellbeing Strategy and Health Inequality Strategy, to bring together a range of services to support a more effective and accessible approach to providing healthcare to our communities. This includes removing barriers to accessing care through outreach approaches tailored to meet the needs of different populations and providing an integrated approach to prevention. MAIN RESPONSIBILITIES: Lead a team of nurses, HCAs and social prescribers conducting health checks, ensuring they deliver evidence-based care, and appropriate clinical investigations and interventions in accordance with our pathways, policies and protocols including: detection of atrial fibrillation, hypertension case finding, cholesterol checks and HbA1C testing, generic health and wellbeing screening for our unregistered population, awareness and education across communities around health risks associated with CVD, AF, smoking, obesity, supporting people to register at a practice, signposting and social prescribing Under the guidance of UKHSA, support the response to infectious disease outbreaks within Herefordshire by undertaking face-to-face clinical assessments and interventions such as swabbing, taking blood samples, administering vaccinations, providing prescriptions or prophylaxis, and identifying and reporting any concerns regarding deterioration in health or requirement for medical intervention or onward referral to GP or secondary care lead for Infection, Prevention and Control within Prevention Services ensuring staff comply with IPC standards and adhere to policies and procedures, and that regular audit and improvement is undertaken, including storage and transportation of all vaccines nurse lead for mass vaccination programmes e.g. COVID-19, ensuring programmes are delivered under a robust governance framework, including the regular review of policies and SOPs, staff training and adherence to PGDs, staff completion and compliance with care immunisation training and vaccine specific updates prior to all seasonal programmes ensure the service actively supports the Systems response to vaccination programmes, ensuring the service is flexible in its approach to delivery, balanced with provision of a safe effective service manage the overall clinical coordination and support Service manager with day to day running of each service area including receipt of vaccines deliveries, confirmation and recording of required vaccine stock, PPE and other consumables such as cholesterol or HbA1C tests regular liaison with other service leads to support the smooth running and functioning of all service areas work closely with the operational manager, the Associate PCN Director for Prevention, Partnerships and Transformation and the Associate Director of Nursing and Quality Improvement to develop a Herefordshire General Practice vaccination vision and strategy support the personal and professional development of staff, ensuring staff are competent in their role through provision of regular training, mentorship, and day-to-day supervision, as well as support with signing of clinical competencies and delivery of clinical supervision sessions upskill staff from the workforce pool to enable them to competently undertake clinical assessment, swabbing, venepuncture and vaccination procedures in support of the Infectious Disease Response Service through the provision of education, training and mentorship confidently utilise a variety of communication skills to manage difficult conversations e.g. sensitive issues, language barriers, hostile behaviour or behaviour that is challenging, with sensitivity and empathy, and convert complex technical information into easily to understand and accessible material Support Service manager to lead Prevention Services clinical governance activities including but not limited to, the management, investigation and learning of patient safety incidents, complaints and concerns, audit programmes, quality improvement projects ensuring staff report incidents via the Datix system, escalating any trends or concerns in a timely manner regular attendance and active participation in the Taurus Healthcare services Clinical Governance meetings, sharing any learning from audits, incidents and complaints with others ensuring the service operates within the set financial parameters, working within the clinical safe work models and under guidance of the Service manager ensuring the safe and effective use of equipment and other resources, ensuring equipment is maintained in good working order, escalation and reporting any concerns or detected faults to the Service Manager ensure vaccine, stationery and health promotion resources are available in immediate work area and monitored appropriately respond to and support staff with any day-to-day clinical queries regarding patient care or escalate to the medical supervisor any beyond own scope of practice escalate any concerns relating to overall clinical care to the Associate Medical Director or Associate Director of Nursing and Quality Improvement as required safeguard people by recognising and responding to an adult or child who might be at risk from abuse but also recognising their own limits and asking for help and escalate concerns when necessary improve and maintain personal and professional development by participating in continuing professional development activities PERSONAL DEVELOPMENT To attend mandatory training and fire lectures regularly and assist with fire policy/evacuation procedure when necessary. To keep up to date with and attend training on revisions to information systems or changes in protocols for the inputting of data in the light of Local and national initiatives. To adhere to agreed protocols at all times. To participate in Individual Performance Review and Personal Development plans on an annual basis and to undertake training and development as identified within these discussions and as indicated by the requirements of the post. CLINICAL GOVERNANCE AND QUALITY ASSURANCE To realise the importance of confidentiality when dealing with patients and staff, particularly when giving or receiving information over the telephone in accordance with the Data Protection Act. Maintain the high standards of the service by contributing towards individual, team and service objectives and acting at all times in the best interest of patients. To accurately collect, collate and input data to Information Systems as required by procedures to ensure that patient database records are up to date and accurate. To participate in Individual Performance Review and Personal Development plans on an annual basis and to undertake training and development as identified within these discussions and as indicated by the requirements of the post. All Information obtained or held during the post-holders period of employment that relates to the business of the company and its service users and employees will remain the property of the Taurus Healthcare. Any breach of confidentiality or computer misuse could lead to disciplinary action, and in serious cases could result in dismissal. Breaches after the post-holders employment has ended could result in the Company taking legal action against them.
