Reablement and Home First Team – Health Care Assistant / Support Worker
The Reablement and Home First Team provides short‑term services in people’s homes to help them regain independence after a hospital stay or illness. The role involves supporting residents with personal care, meal preparation, medication administration and exercise programmes.
Key Responsibilities
* Support people with personal care, meal preparation and medication administration
* Supervise exercise programmes and assist with mobility
* Assist with general daily living activities and promote independence
* Participate in reflective practice and regular training sessions
* Maintain accurate and timely documentation of care provided
Shift Pattern and Working Hours
The service operates 7 days a week between 07:00 and 22:00. Full‑time and part‑time positions are available on a rolling rota (4 on/4 off or 2 on/6 off). Desired shifts include AM 08:00–13:30 (4 on/4 off), PM 17:00–22:00 (4 on/4 off) and split shifts for full‑time posts.
Key Requirements
* Unrestricted right to work in the UK
* Valid UK driving licence and access to a vehicle for local visits
* Level 3 Diploma in Clinical Healthcare or willingness to undertake an apprenticeship alongside the post (optional but valued)
Qualifications & Experience
Essential:
* Experience of working with people, families and carers in challenging situations
* Confident in using IT systems and willingness to be trained on specific systems
* Evidence of literacy and numeracy skills – GCSE pass or equivalent to Level 2 functional skills Maths and English
* Care Certificate (required for the role)
Desirable:
* NVQ in a relevant topic (e.g. healthcare) or other related training
* Experience working in a community or in people’s own homes
* Experience providing personal and intimate care to people in a health or social care setting
* Practical experience motivating people and encouraging independence
