The post will include care coordinating a designated caseload, working within CPA Policy and associated countywide risk procedures, travelling independently, use of IT skills and a working knowledge of legislation regarding safeguarding vulnerable adults, the MHA and MCA.
Social Work Specific:
* Arranging the provision or purchase of services to meet the assessed needs of individuals and their carers in more complex cases, whether within Gloucestershire or at a distance and to reassess in line with Care Act requirements.
* Undertake the statutory strengths based assessments to facilitate personalised provision of care and support.
* Working in accordance with statutory and legal requirements and offering specialist knowledge and consultation to the team on the Care Act, Mental Health Act, Human Rights Act, Housing Act and a range of social care policy issues including appropriate adult and property protection.
* Social Work provision within GHC Trust is currently undergoing service development to strengthen the Social Care management and service provision whilst maintaining integration alongside the development of a new Social Care Hub.
