Overview
Community Occupational Therapist role at Cheswold Park Hospital. Band 6, 37.5 hours per week. Permanent full-time position within the Community Enhancing Recovery Team (CERT).
Responsibilities
* Work as an integral member of the multi-disciplinary team providing specialist occupational therapy assessment and treatment for service users in receipt of care from community services.
* Create care plans with the MDT and work closely with the OTA and support workers to deliver care.
* Use assessment skills and specialist knowledge to prevent admission and implement rehabilitation at home away from locked rehabilitation units.
* Develop and evaluate individual and group OT interventions, using standardised and non-standardised assessments and outcome measures.
* Maintain clinical records and contribute to care reviews, referrals, case conferences and care programme approach meetings.
* Provide reports and data via electronic recording systems; participate in supervision and provide supervision to others.
* Operate as an autonomous practitioner and participate in service governance, audit, and continuing professional development.
Qualifications
* Essential Diploma/degree in Occupational Therapy; HCPC registration.
* Desirable Leadership/first line management certificate.

* Ability to work autonomously, manage complex information, and build relationships within the MDT and community partners.
Experience and Training
* Essential Post-registration experience in mental health or learning disabilities; ongoing CPD; supervision of staff/students.
* Desirable Leadership experience; post-graduate training in mental health (e.g., psychosocial interventions, CBT/counselling, standardised assessments).
* Practice Education/Practice Assessor Preparation Certificate or willingness to achieve within 12 months.
