Children's & Joint Commissioning Services need Sessional Family Support Worker's to assist within the newly created Edge of Care Team.
You will work alongside Social Workers, other agencies and the family network. You will focus on children and families who require intensive support to prevent placement breakdown and to re-stabilise the family unit.
You will provide a range of interventions to children and families and work intensively to support children and families to prevent and reduce the number of children coming into our care. For example; direct work with parents/children/young people; provision of transport/escorting children; working with families in their homes including assistance with day-to-day living needs, delivering parenting packages, crisis intervention and promoting positive family relationships.
You will also be required to write detailed legible case notes, write reports relative to the position and update the electronic child's record in a timely manner.
You will have access to your own vehicle and be able to use this vehicle at all times. Business insurance is essential.
