FEP/EIP Care Coordinator (Newly Qualified Social Worker/Nurse)

Are you a qualified Social Worker or Mental Health Nurse with a passion for mental health care? We're looking for compassionate professionals to join our Early Intervention in Psychosis (EIP) Service – PATH (Psychosis: Prevention, Assessment and Treatment in Hertfordshire).
Based in Stevenage, covering a caseload in the South East of the county, this is a rewarding opportunity to support people experiencing psychosis for the first time, delivering personalised and recovery-focused care.
You will hold a caseload and work closely with the wider multi‑disciplinary team, developing leadership and delegation skills.

Main duties of the job
* Active member of a skilled multi‑disciplinary team
* Provide the PATH service in a designated locality
* Work in community settings, both in people's homes and local hub buildings
* Train as a family intervention therapist
* Deliver recovery‑based and collaborative approaches to care planning, working flexibly and creatively with people to achieve their individual goals
Job responsibilities
You will be responsible for the clinical and operational day‑to‑day service delivery, providing a high‑quality health and social care service to adults of all ages having First Episode Psychosis (FEP). Key activities include assessment, care planning and care coordination within a recovery framework.
Person Specification – Education / Training
* Degree level or equivalent academic qualification (Diploma/Degree in Mental Health Nursing, Social Work, Occupational Therapy)
* Registration with NMC / HCPC / SWE
* Advanced training in mental health
* Approved Mental Health Professional / Best Interest Assessment training
Experience and Knowledge
* At least 2 years of post‑registration experience, including at least one year at Band 5 (marginally less experience may be considered if competence is demonstrated)
* Experience working in acute mental health inpatient/day‑care services
* Experience in crisis and emotionally demanding environments
* Knowledge of safeguarding, family breakdown, serious mental ill health and risk of self‑harm
* Experience of partnership working with service users and carers, demonstrating dignity and respect
* Experience of teaching and training
* Excellent communication skills with a wide range of stakeholders, verbally and in writing
* Experience using electronic recording systems and producing accurate, timely reports
Physical Skills
* Car driver essential, or access to a car (unless a disability prevents driving)
* Full UK driving licence
* Business insurance to use car for work purposes
