Bridge End Surgery, Chester le Street, has a vacancy for an experienced Practice Nurse with Chronic Disease Management, particularly Asthma and COPD to join our busy and friendly Nursing Team.


Main duties of the job

Practice Nursing duties will include but not exhaustive

Chronic disease management

Cytology

Vaccination and Immunisations

Childhood immunisations

Wound care

Family Planning

Working within practice policies and NICE guidelines


About us

We are a well established Practice based in Chester le Street and are part of Chester le Street Primary Care Network with a population of approximately 8000 patients

The clinical team currently comprises of 2 GP Partners, 6 salaried GP's, 1 Advanced Nurse Practitioner, 2 Practice Nurses, 1 Health Care Assistant, 1 Nurse Associate and 1 GPA. We also have a very experienced Reception and Admin Team.

Bridge End Surgery are an established training practice.


Job responsibilities

Working under supervision of the Lead Nurse, the Practice Nurse will work as part of the Nursing team providing General Medical Services to all our patients

Responsibilities

Implement and evaluate treatment plans for patients with a known long term condition

Identify, and manage as appropriate, treatment plans for patients at risk of developing a long term condition

Advising patients with respect to their continuing nursing and medical needs.

Assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on heath and well-being

Ensure clinical practice is safe and effective and remains within boundaries of competence and to acknowledge limitations tot eh team so that referrals can be appropriate

Support patients to adopt health promotion strategies that promote patients to live healthily and encourage principles of self-care

Assess and care for patients presenting with uncomplicated wounds

Perform routine procedures like ear syringing, treating wounds, and applying or removing dressings

Taking urine and blood samples, other swabs and specimens

Support and advise women requesting information relating to family planning

Support and manage health needs of women presenting for cervical cytology

Implement and participate in vaccination and immunisation programmes for both adults and children

Keeping own work areas and general/patient areas generally clean, identifying issues and hazards/risk in relation to other work areas within the business.

Undertake periodic infection control training

Training and personal development

Training requirements will be monitored by appraisal initially after 6 months and then yearly and will be in accordance with Practice requirements. Personal development will be encouraged and supported by the practice, where appropriate. It is the individuals responsibility to remain up-to-date with recent developments

Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development ensuring requirements are met.

As well as the nursing team there is a need to work closely with reception, admin and clinical staff to ensure smooth running of the practice, reporting any problems encountered to the relevant person and ensuring everyone is aware of the different roles within the nursing team

In the course of seeking treatment, patients entrust uswith, or allow us to gather, sensitive information in relation to their healthand other matters. They do so in confidence and have the right to expect thatstaff will respect their privacy and act appropriately.

In the performance of the duties outlined in this jobdescription, the post-holder may have access to confidential informationrelating to patients and their carers, practice staff and other healthcareworkers. They may also have access to information relating to the practice as abusiness organisation. All such information from any source is to be regardedas strictly confidential.

Information relating to patients, carers, colleagues, otherhealthcare workers or the business of the practice may only be divulged toauthorised persons in accordance with the practice policies and proceduresrelating to confidentiality and the protection of personal and sensitive data.

The post holder will assist in promoting and maintainingtheir own and others health, safe and security as defined in the PracticeHealth and safety policy, to include:

Using personal security systems within the workplaceaccording to practice guidelines

Identifying the risks involved in work activities andundertaking such activities in a way that manages those risks across clinicaland patient process

Making effective use of training to update knowledge andskills, and initiate and manage the training of others across the full range ofinfection control and patient processes

Using appropriate infection control procedures, maintainingwork areas in a tidy, clean and sterile and safe way, free from hazards.Initiation of remedial / corrective action where needed or escalation toresponsible management

Participation in an annual individual performance review,including taking responsibility for maintaining a record of own personal and/orprofessional development

Taking responsibility for own development, learning andperformance and demonstrating skills and activities to others who areundertaking similar work.


Person Specification


Experience

* Experience with Chronic disease management.
* Experience working autonomously.
* Experience working in Primary Care/General Practice


Qualifications

* Registered Nurse (Nursing & Midwifery Council)
* Diplomas in Chronic Disease Management
* Knowledge of QOF
* Experience of Infection, Prevention and Control
* Knowledge of EMIS
