This position provides personal care and paraprofessional services according to an established plan of care. Responsibilities include housekeeping, meal preparation, companionship, and maintaining a safe, clean environment.

Essential Functions
* Assist with activities of daily living: personal hygiene, ambulation/transfers, eating, dressing, shaving
* Prepare meals and snacks as per instructions
* Provide client-specific activities: hospice care, range of motion exercises, diet management, blood glucose monitoring, vital signs, medication reminders
* Perform homemaking tasks: vacuuming, dusting, cleaning, laundry, etc.
* Run errands or provide transportation as needed
* Live in the client’s home for 4-7 days per week, either temporarily or permanently
Preferred Experience
* Personal care
* Range of motion exercises
* Companionship
* Light cleaning
Qualifications
* High School diploma or G.E.D. preferred
* Experience as CNA, HHA, or PCA in home or facility settings
* Completion of a state-approved Nurse Aide, HHA, or PCA program
* CPR certification recommended
* Proficiency in English (reading, writing, speaking)
* Valid driver’s license and access to transportation
* Patience and passion for assisting elderly and disabled adults
