Job overview
A registered practitioner (nurse or paramedic) with post‑registration practice at Band 5, experienced in multidisciplinary and multi‑agency working, is required to triage and manage an urgent caseload on a Virtual Ward, acting as a step‑up or step‑down to community services. The role involves collaboration with adult services, care navigation teams, social services, and acute trusts to keep patients in their homes and reduce avoidable admissions.
Main duties
* Assess referrals and implement enhanced care programmes within the Virtual Ward.
* Provide high‑standard clinical judgement and triage support for service users accessing community services.
* Coordinate and deliver quality clinical care programmes in line with Trust policies and procedures.
* Work effectively within a multi‑agency, multidisciplinary team adopting a no‑blame culture.
* Adhere to Trust policies, procedures, and protocols to promote quality patient experience.
* Maintain sound knowledge of Community Services.
Person specification
Qualifications
* Registered Nurse or Paramedic Level 1/2.
* Diploma level education or evidence of substantive post‑registration Band 5 experience in a similar primary care setting.
* Evidence of personal and professional development.
Experience
* Multi‑disciplinary and multi‑agency working experience.
* Post‑registration practice within a secondary or primary care setting.
* In‑depth knowledge of Virtual Ward requirements.
* Knowledge of NHS policies affecting hospital avoidance.
* Knowledge of clinical governance.
