About the Team
The Bath Social Care Team is looking for passionate individuals to join a team focused on excellent social care. As an Adult Social Care Community Team, we work with a wide range of people with varying needs. This provides excellent opportunities for developing social work skills and knowledge. The team is experienced, supportive, and reflective, consisting of social workers, social care practitioners, occupational therapists, and administrators.
About the Role
As an Adult Care Social Care Practitioner, your work will be rewarding. You will undertake assessments and support plans with people who have physical disabilities or other complex needs. This role involves working with families, carers, professionals, agencies, and organizations across Bath's World Heritage site.
The focus is on promoting independence, enabling individuals to remain in their homes, and reducing hospital admissions or long-term care. The ideal candidate will have experience in health and/or social care, knowledge of the Care Act, and the ability to apply it practically. You should understand what matters to service users, work closely with the community and partners, and support individuals positively and responsively. Commitment to maintaining professional standards and using resources creatively, including assistive technology, is essential.

About You
To join us, you must have: A relevant qualification equivalent to NVQ Level 3 or equivalent health and social care experience.
Experience in health and social care assessments, caseload management, and good interpersonal, verbal, written, and IT skills are required.
