Adult’s Social Worker
Team: Hospital Discharge Mental Health
Location: Sheffield
Rate: £30
The successful candidate will need to be an experienced Social Worker with a mental health background.
Core tasks and duties as follows:-
– Undertaking social care assessments and delivering timely care packages for hospital inpatients prior to discharge
– Some mental capacity assessments, BI decisions and safeguarding as required
– Understanding of Section 117, Attending 117 meetings, ward rounds, professional meetings
– Participation in duty rota
