Job Description
Wrexham County Borough Council is seeking to recruit an experienced care broker to source and negotiate residential and nursing care home placements for adults with assessed health and social care needs. The role involves accurate and timely processing of care plan information and liaising with providers and professionals to source appropriate care for individuals in Wrexham. This is a temporary, full-time position initially for 3 months, with potential for extension, offering a pay rate of £15.84 per hour. The role supports hybrid working, with an expectation to be based in the office 2 days per week.
Responsibilities include:
1. Broker care packages and placements for adults, ensuring support is personalized and tailored.
2. Negotiate with care providers to secure placements that meet individual needs and budget.
3. Facilitate the timely discharge of patients from hospitals, including outside normal hours.
4. Liaise with social care practitioners to confirm service requirements and outcomes.
Ideal candidate will have:
* Enhanced DBS check.
* Experience in Health and Social Care.
* Experience in commissioning, brokerage, or purchasing.
* Ability to work independently and within a team.
