Job summary

We seek newly qualified or experienced nurses to provide complex care and treatment to adults with long-term or complex health needs in the community.

Hampshire and Isle of Wight Healthcare NHS Foundation Trust offers excellent learning and development opportunities, including a comprehensive preceptorship program with an experienced coach to support your confidence and competence.

You will manage your allocated workload daily, with opportunities to develop skills in shift coordination and line management of junior staff. Access to a vehicle and a full driving license are essential.


Main duties of the job

The post-holder will manage a defined community caseload, working within professional guidelines and accountability, providing interventions through Nursing, Mental Health, Physiotherapy, or Occupational Therapy. Responsibilities include ensuring referrals are handled appropriately, conducting holistic assessments, and maintaining accurate care plans in line with standards.

The role requires working autonomously with professional accountability, aiming to prevent unnecessary hospital admissions and support timely discharges. Responsibilities also include re-enablement, rehabilitation, health promotion, and reducing health inequalities.

Key responsibilities include providing compassionate, high-quality care, effective communication, clinical competence, holistic assessment, safeguarding, medicines management, and maintaining professional standards. You will develop and maintain collaborative relationships within multidisciplinary teams, promote person-centered practice, and stay updated with clinical developments.

You will participate in discharge planning, promote safe clinical practices, and ensure accurate record-keeping. Responsibilities also include managing conflicts, supporting students and new staff, engaging in clinical audits, and contributing to service improvements.

The role involves clinical procedures such as injections, wound management, and catheter care, with the ability to travel across Trust sites. You will make autonomous clinical decisions, prioritize patient care, and be accountable for your actions and those of the team.


Person Specification


Qualifications

* BSc Nursing or RN registered with NMC
* Post-registration qualifications as applicable


Experience

* Working with multidisciplinary teams and agencies
* Handling sensitive issues
* Planning and organizing resources and patient needs
* Managing workload independently
* Effective communication skills
* Knowledge of frailty, long-term condition management, re-enablement, palliative care


Additional Criteria

* Full UK driving license is required
* Applicants with non-GB licenses cannot be considered for community roles
