Offer description

* The post holder will work within their clinical competencies to provide specialist pharmaceutical supportto thepractice as part of a multi-disciplinary team to improve care and support safe and effectiveprescribing.
* The role will have a patient facing element either face to face or telephone - including chronic disease management, repeat prescription management and structured medication reviews. This will also include the management and review of patients with polypharmacy
* The post holder will need to be proactively involved in quality improvement and clinical audits as well as managing some aspects of the Quality and Outcomes Framework to deliver an excellent service within general practice


Main duties of the job

* Clinical medication reviews for patientsboth in person and via telephone, with single or multiple long term conditionswhere medicines optimisation is required
* To reconcile medicines following discharge from hospitals or intermediate care and working with patients andcommunity pharmacists to ensure patients receive the medicines they needpost-discharge and to reduce risk of readmission
* To manage repeat prescription requests,resolving queries where possible within scope of practice. To ensure patientshave appropriate monitoring tests in place when required.
* Implement changes to medicines in linewith MHRA alerts, product withdrawal or shortage and other local or nationalguidance
* To undertake a proactive role in auditand quality improvement implementing recommendations where appropriate.
* Provide independent information topatients regarding medicines and prescribing changes, initiating furthersupport from other healthcare professionals where appropriate
* To identify areas of clinical risk at networklevel and make recommendations to support the introduction of new workingpractices that will optimise the quality of prescribing and make more efficientuse of network resources.
* Work with the practice to ensure fullcompliance with Care Quality Commission standards for safe and effective care

We are looking for a dedicated Clinical Pharmacist to join and enhance our team.


Job responsibilities

To be involved in multidisciplinaryclinical meetings within the practice where appropriate

To manage repeat prescriptionrequests, resolving queries where possible within scope of practice. To ensurepatients have appropriate monitoring tests in place when required.

Implement changes to medicines inline with MHRA alerts, product withdrawal or shortage and other local ornational guidance

To maintain full and completerecords of all patient contacts using appropriate clinical templates and coding

To undertake a proactive role inaudit and quality improvement implementing recommendations where appropriate.

Provide independent information topatients regarding medicines and prescribing changes, initiating furthersupport from other healthcare professionals where appropriate

Participate in review and settingof policies relating to medicines management and prescribing and help ensurepractices prescribe in accordance with local guidelines and formulary

To identify areas of clinical riskat network level and make recommendations to support the introduction of newworking practices that will optimise the quality of prescribing and make moreefficient use of network resources.

To maintain own clinical andprofessional competence and be responsible for own continuous professionaldevelopment

Work with the practice to ensurefull compliance with Care Quality Commission standards for safe and effectivecare

Other requirements

* Meets DBS reference standards and has a clear criminal record, in line with the law on spent convictions
* Adaptable
* Evidence of being a great team-player
* Self-Motivation
* Safeguarding and other mandatory training
* Access to own transport and ability to travel across the locality on a regular basis

Qualifications

* Master of Pharmacy (MPharm) degree, which is accredited by the General Pharmaceutical Council (GPhC) + 1 year pre-registration training.
* Demonstrates and understanding of, and conforms to, relevant standards of practice.
* Follows professional and organisational policies/procedures relating to performance management
* Awareness of the breadth of common acute and long-terms conditions that are likely to be seen in general practice
* Non medical prescriber status
* Membership Primary Care Pharmacy Association (PCPA)
* Membership of the Royal Pharmaceutical Society


Experience

* Knowledge of IT systems, including ability to use word processing skills, emails and the internet to create simple plans and reports
* Able to gain acceptance for recommendations and influence/motivate/persuade the audience to comply with the recommendations/agreed course of action where there may be significant barriers
* Experience of working in primary care / general practice
* In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare
* Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities
* An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing. Analyse and interpret complex prescribing information and present information in a clear way
* Able to plan, manage, monitor and review general medicine optimisation issues in core areas for long term conditions
