We achieved the status of being a Research Practice in July 23 and nowdeliver several research studies across Meridian PCN with a highly active andexperienced team. We support portfolio nurses with extra roles. The partnersthemselves are skilled in education and research.

Person Specification
Qualifications
* Registered Nurse (NMC)
* Evidence of ongoing CPD
* Independent Non Medical Prescriber
* Primary Care Nursing qualification
Experience
* Minimum 2 years experience managing Type 1 & Type 2 diabetes in primary, community or secondary care
* Experience running autonomous clinics
* Experience using SystemOne and Microsoft Office
* Experience contributing to governance and audit or quality improvement
* Experience managing other long term conditions
* Experience supporting multi-morbid and housebound patients
Skills & Knowledge
* Strong clinical knowledge of diabetes management, including medication titration
* Ability to educate and mentor colleagues
* Excellent communication and interpersonal skills
* Ability to prioritise and manage a busy caseload
* Strong IT skills and accurate record keeping
* Understanding of evidence based practice and PGDs
