Job Summary


We would consider offering this as a preceptorship to Band 7 psychologists currently in CAMHS over a 18 month-2 year period depending on experience.

This is one of two Transition Worker posts based within our Tier 3 Team at Merton CAMHS. The focus is on supporting young people to transition from Child and Adolescent Mental Health Services (CAMHS) to Adult Services. The post holder will also support families and work alongside teams and liaise with wider organizations ensuring wider system collaboration to wrap around support for our young people.

The post holder will also support families and work alongside teams and liaise with wider organizations ensuring wider system collaboration to wrap around support for our young people.

These roles have been developed to ensure our young people moving from CAMHS services into adult services are sufficiently prepared and supported to navigate adult provision and networks, whilst maintaining their progress, promoting independence and enhancing social and mental health outcomes.

Main duties of the job

It is acknowledged that the transition from child to adult services creates risks for young people who may fall through the systems due to not having the right support. This can result in poor mental health and social outcomes, leading to loss of human potential and increased burden on health and social care services over a lifespan.

This post will directly inform and update Transition Protocol between CAMHS and AMHS based upon the latest evidence of best practice. The post holder will also be responsible for supporting teams and provide training, where necessary, to educate and skill up professionals and stakeholders in the needs of those young people moving from CAMHS into adult services. Core to the role with be ensuring excellent collaboration between the young person and families to support the young person's journey across services.

We are looking for an individual who has a strong track record in system wide working and communication across teams, who demonstrates excellent leadership qualities and someone who is inspirational and engaging with young people, staff and external partners.

Merton Tier 3 and SPA teams are based at Birches Hose, Mitcham a beautiful building overlooking the cricket green and close to local shops and transport links. Both are friendly, supportive multidisciplinary teams who enjoy close working relationships.

Person Specification

Experience

Essential

* Experience of specialist psychological assessment and treatment of clients across the full range of care settings, including outpatient, community, primary care and in-patient settings
* Experience of working with a wide variety of client groups, across the whole life course with presenting problems that reflect the full range of clinical severity including maintaining a high degree of professionalism in the face of highly emotive and distressing problems, verbal abuse and the threat of physical abuse
* Experience of working with young people who have experienced severe, complex and long-term mental health problems
* Experience of working with young people 16-25 years old and their families
* Demonstrate further specialist training/experience through having received a minimum of 50 hours clinical supervision while working as a specialist clinical psychologist over a minimum of 18 months, or an alternative agreed by the Director of Psychology

* Experience of teaching, training and/or supervision
* Experience of the application of clinical/counselling psychology in different cultural contexts
* Experience of working as part of a multidisciplinary team
* Experience of working within a multicultural framework
