Band 5 Staff Nurse-HCOP

Ward 11 are seeking a passionate, enthusiastic and committed staff nurse to join our dedicated and dynamic team. If you have passion and drive within geriatric medicine and would take pride in delivering compassionate care to this patient group, this would be a great opportunity for you to strive with the support of an experienced multi disciplinary team. The ideal candidate would be expected to deliver the highest standards of care to our aged patients, including those experiencing symptoms of delirium and during different stages of dementia. A keen interest in falls prevention, dementia care, tissue viability and nutrition and hydration and end of life care are vital to ensure staff are able to deliver holistic care to the older adult and opportunities to expand knowledge within these areas will be supported. As well as being able to deliver care using a skilled approach, staff will gain vast amounts of experience in complex discharge planning. Excellent communication skills and a desire to improve patient care are essential for this post.

This opportunity will not only enable the successful candidate to gain experience in the above areas but also in acute medicine. There are opportunities on a daily basis to nurse patients with various health conditions allowing the clinical setting to function at a fast pace with an abundance of learning opportunities.


Main duties of the job

This post would be ideal for experienced nurses and nurses considering returning to the NHS, however newly qualified, junior applicants would be considered.

Carry out clinical practice within designated clinical areas, ensuring that high quality current evidence based care underpins all actions and interactions with patients

Be responsible for completing/ undertaking relevant nursing/ clinical procedures and practice underpinned by theoretical knowledge and practical experience

cUse up to date knowledge and evidence to assess, plan, deliver and evaluate care, communicate findings, influence changes and promote health and best practice. The individual must make person centred, evidence based judgements in partnership with others involved in the care process to ensure high quality care

dAct as advocate for patients within the areas, to ensure a patient focused approach to the delivery of care

Ensure patients receive high quality clinical care and a positive patient experience, having regard to their customs, beliefs and ethnicity

fPromote a clean and safe environment for staff, patients and visitors by ensuring compliance with legislation /policies and procedures, health care associated infection prevention, governance including risk management and incident reporting and act without delay in the reporting and escalating of any areas of concern

gRecognise and act to avoid situations that may be detrimental to the health and wellbeing of patients

Person Specification


Qualifications

* * Evidence of professional and clinical knowledge acquired through training to degree/diploma level leading to Professional registration on the appropriate part of the NMC register i.e.
* ( Adult / Child / Learning Disability or Mental Health)
* * Evidence of undergraduate education for newly qualified nurses / or post graduate continuing professional development for registered nurses who have post registration experience


Experience

* * Previous / recent experience or working in an acute secondary health and care setting (can include undergraduate student placements within the last 12 months)
* * Willingness to become a practice supervisor / assessor to or meets Nursing and Midwifery Council (NMC) standards for student supervision and assessment
* * Demonstrable understanding and knowledge of the NMC code of conduct and the proficiencies and standards for registered nurses can apply this to undergraduate training or post graduate practice
* * Being an accountable professional
* * Promoting health and preventing ill health
* * Assessing needs and planning care
* * Providing and evaluating care
* * Leading and managing nursing care and working in teams -- including prioritisation and delegation skills
* * Coordinating care and responding to deteriorating health
* * Improving safety and quality of care
* * Awareness of current relevant clinical and professional issues and NHS policy
* * Understanding / experience of the principles of Safeguarding and Mental Capacity
* * Awareness of Health and Safety issues.
* * Experience in working in an Acute NHS practice setting as an undergraduate student nurse or post graduate registered nurse.
