Job summary
Riverside Medical Practice arelooking for a motivated and enthusiastic Practice Nurse to join our friendly,patient focused practice team. Chronic Disease management skills are imperativeto the role.
The Practice Nurse will beresponsible for a number of clinical areas such as infection prevention and control,health promotion, chronic disease management, immunisations, cervicalscreening, as well as actively supporting the practice management team in thereviewing and delivery of clinical policy and procedures.
The Practice nurse will also beresponsible for the effective management of patients with long-term conditionsusing evidence-based practice including care for elderly and houseboundpatients.
Main duties of the job
The main duties of the role are:
1. Health promotion and wellbeing
2. Implement and evaluate individual treatment plans for chronic disease patients.
3. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on patients' health
4. Provide routine nursing care to patients as required in accordance with clinical-based evidence, NICE and the NSF
5. Provide wound care (ulcer/Doppler etc.) to patients
6. Provide immunisations (childhood, seasonal etc) to patients
7. Provide travel medicine services
8. Provide Cervical Cytology screening
9. Request and process pathology results as required
10. Maintain accurate clinical records in conjunction with existing legislation
11. Ensure read codes are used effectively and support with achievement of QOF targets
12. Chaperone patients where necessary
13. Assist GPs with minor surgery when required
14. Support the team in dealing with clinical emergencies and safeguarding matters.
15. Support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice)
16. Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

43. 3) Experience of infection prevention and control.
44. 4) Experience of understanding the importance of evidence based practice
