The Enhanced Nurse role will in relation to delivering health services, and developing the nursing and GPA team, including line management responsibilities. Please see the full job description for detail.

Main duties of the job
Enhanced practice nurses have the following key responsibilities in relation to delivering health services:
* Works as part of a MDT within the PCN; consults with patients, their family, and the multi-professional team to undertake assessments of patient need and devise and evaluate complex care plans.
* Teaches and advises patients and their families on how to manage their condition or support the multi-disciplinary team to do so.
* Recognises boundaries of its practice and knows when and to whom patients should be referred.
* Holds responsibility for team performance and service delivery.
* Facilitates and supports the development and delivery of innovative training and education to staff, to improve competence and confidence, in line with best practice and guidance related to area of expertise, and participates in and/or leads formal training events promoting best practice in area of expertise.
* Provides professional and clinical leadership and mentorship, acting as a clinical role‑model on the delivery of evidence‑based practice.
* Advises and supports on the development and delivery of work streams relating to area of expertise as part of the PCN’s strategy, where appropriate.
* Contributes to the delivery of local policy, procedures and protocols for the PCN to improve identification, assessment, ongoing support for patients and their family carers.

Experience
* Working at an enhanced level of practice as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework.
* Experience of working within general practice and PCNs.
* Evidence of personal development.
* Experience of nurse-led management of chronic conditions.
* Experience of implementing protocols and clinical guidelines.
* Experience of quality initiatives such as clinical benchmarking.
* Specific General Practice skills and qualifications.
* Excellent communication and interpersonal skills.
* Ability to work as part of a multi-disciplinary team.
* Ability to work independently.
* Ability to organise and prioritise workload effectively.
* Ability to exercise sound judgement when faced with conflicting pressures.
* Ability to handle a busy and varied primary care workload.
* Ability to offer support within a clinical team and share learning points in team meetings.
* Understanding of and commitment to upholding the Nolan Principles of Public Life.
* UK driving licence.
* Experience of working with other primary care providers; OOH/EA.
* An awareness of current developments and initiatives in primary care.
* Understanding of the pressures faced by practices and healthcare teams.
* Knowledge of the pathways and protocols in existence.
Qualifications
* Registered with the Nursing & Midwifery Council.
* Evidence of other post registration at level 7 or above relevant to their area of enhanced practice.
* Specialised interests qualification.
