About The Role
We have an exciting vacancy for a Children with Disabilities Senior Practitioner in the Children with Disabilities Support Services. This role provides short break sessions in Early Support Learning provision for children aged 0-5 years and Targeted Positive Activities for children and young people aged 0-18 years.
Sessions and groups focus on high-quality targeted early educational learning and support for children with disabilities across Lincolnshire. The role involves leading in the Targeted Positive Provision for children aged 5-18 years, providing leadership and management for a team of practitioners, and offering a short break for parents and carers.
The service operates in evenings, Saturdays, and during school holidays, supporting children and young people with disabilities or complex health needs to be happy, healthy, and safe.

Main Duties
* Facilitate high-quality sessions for children with disabilities, supporting their development and providing respite for families.
* Plan sessions aligned with the EYFS Curriculum, focusing on closing the attainment gap and meeting health and personal care needs.
* Assess and monitor children's progress, using tools like Tapestry and Outcome Wheels.
* Support families to access advice, local services, and peer networks.
Essential Criteria
* NVQ Level 3 in Early Years/Childcare, Youth/Playwork, or relevant experience working with children with disabilities.
* Understanding of individual needs of disabled children and young people.
* Effective communication skills.
* Experience working with multi-agency teams.
This is a fixed-term position until March 2027, with 28 hours per week, working 52 weeks annually. Virtual interviews will be scheduled via Microsoft Teams. All applicants must register with the DBS Update Service and undertake a new DBS check.
