We are looking for a caring, dedicated and reliableindividual to join our team as a non-clinical care coordinator supporting thewider pharmacy team. This is an important role within the PCN, focusing on usingsystems and data to proactively identify patients in need of routine drugmonitoring, alongside coordination and navigation of care.


Main duties of the job

The main responsibilities includebut are not limited to the below:

Work alongside clinicalpharmacists to coordinate blood tests for patients

Contact patients to arrangeappointments for drug monitoring and follow-up reviews

Run searches to identifypatients requiring tests and maintain a tracking system to ensure they attendwithin appropriate timeframes

Identify and follow-up withpatients who miss appointments or are overdue for monitoring.

Communicate clearly withpatients regarding appointment details, preparation instructions and anynecessary reminders in a professional, timely and supportive manner.

Liaise with the widermultidisciplinary team as and when required to provide holistic care forpatient.

Collaboratewith the prescriptions team to process online requests, manage prescriptionrelatedtasks and emails, and liaise with the pharmacy team to ensure accurate andefficient prescription processing.

JOB DESCRIPTION

JOB TITLE: Non-Clinical CareCoordinator (Medications Management Team)

HOURS: 30 hours per week(working pattern to be agreed)

Job Summary:

We are looking fora caring, dedicated and reliable individual to join our team as a non-clinicalcare coordinator supporting the wider pharmacy team. This is an important rolewithin the PCN, focusing on proactive identification of patients in need of routinedrug monitoring, alongside coordination and navigation of care.

You will workclosely with the clinical pharmacists and the wider medicines management teamand act as a central point of contact for patients. Your primary responsibilitywill be to support clinical staff by ensuring timely patient communication,accurate record-keeping and a smooth workflow for the medication monitoringprocess.

You will alsoreceive training as a prescription clerk to ensure you gain a comprehensiveunderstanding of our systems and are able to provide support to theprescriptions team when needed.

This is a nonclinicalrole with minimal patient contact and requires strong IT proficiency and welldevelopedorganisational skills.

To monitor service provision to ensure thatservices are being delivered in line with contract.

Person Specification


Experience

* Experience and ability of working to policies and procedures
* Experience of working in a multi-professional team environment
* Experience of working in an administrative capacity
* Experience of working with data and using this to produce lists to work from.
* Knowledge of clinical systems used by medication management teams.
* Experience of working directly in care coordinator role, adult health and social care or public health.
* Knowledge of what a PCN is and how it operates.


Skills & Abilities

* Candidates should be able to:
* Communicate effectively, both verbally and in writing, with people, their families and carers
* Identify and communicate risks if they arise whilst working with individuals
* Organise, plan and prioritise tasks using own initiative, including when under pressure and meeting deadlines.
* Work flexibly and adapt to new challenges with a positive attitude.
* Use MS Office effectively, including Word, Excel and Outlook
* Collect data effectively and use tools to measure impact of services
* Have experience of using clinical systems.
* Hold qualifications / professional development in Microsoft 365 and clinical systems.


Qualifications

* Demonstrate commitment to professional and personal development, and enrol, undertake or be qualified from appropriate training by the Personalised Training Institute (successful candidate will receive training unless already qualified)
* Grade 4 / Grade C or above in English Language and Maths
* Level 3 qualification (NVQ, BTEC, A-Level) or higher in health and social care or equivalent.
* Experience or training in personalised care and support planning
