We are offering an exciting opportunity for a committed& enthusiastic Practice Nurse to join our friendly & well-establishedteam.
The position is for 28 hours across four days, although there may be someflexibility afforded for the right candidate. Experience of practice nursing ishighly desirable but not essential as further training and support would beavailable to the right candidate. The salary is negotiable, dependent on levelof experience and qualification.
Credas Medical provides primary care medicalservices to approximately 8600 patients across three sites in the DerbyshireDales. You will be based at our main site in Darley Dale; a modern,purpose-built surgery close to local shops and Whitworth Park.
We have twobranch surgeries situated in the nearby villages of Winster and Youlgreave,where you may be asked to cover in times of sickness or annual leave.
Main duties of the job
The post holder will be responsible for the delivery of general practice nursing to the whole practice population. The focus of the role is both the delivery of evidence-based practice for patients presenting with a long-term condition, and the provision of preventative healthcare to the practice population.
As an autonomous practitioner, the Practice Nurse will be responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. They will work collaboratively within the general practice team to meet the needs of patients, support the delivery of policies and procedures, and provide nurse leadership as required.

Credas Medical covers a predominantly rural area and lies North of Matlock within the Derbyshire Dales district. We are a friendly and dedicated team, many of whom have worked within the Practice for many years. We are a training practice with a strong culture of learning and development. The medical team consists of four GP partners, five salaried GPs, two GP registrars and two F2 Doctors.
The nursing team comprises of six Practice Nurses and four Health Care Assistants. Clinical work is supported by PCN staff in the roles of physiotherapist, pharmacist, paramedic, mental health worker and ACP. The clinical team are supported by a highly skilled and dynamic group of management and administrative staff.
Job responsibilities
Role Purpose
* To work as an autonomous practitioner, providing care to patients within the primary care setting using specialist practice nurse skills.
* To work collaboratively with the whole practice team, ensuring that the care delivered to the practice population is relevant, responsive, of the highest standard and is clinically effective and efficient.
* To work within national, local and practice policies, protocols and guidelines, whilst using experience and clinical judgement to provide nursing care.
* To work within the framework of the NMC Code of Professional Conduct and Scope of Professional Practice to ensure that the highest standard of care is given to the practice population.
* Contribute to improving quality standards through participation in staff meetings, clinical supervision and audit.
Job responsibilities
Provide chronic disease management for Diabetes, COPD, Asthma and CHD/CVD, maintaining robust call and recall systems.
Participate in national screening programmes such as cervical screening and NHS health checks.
Deliver the routine immunisation schedule for children and adults, as well as selective immunisation programmes as defined by UKHSA.
Undertake routine procedures such as venepuncture, ECG and clinical observations.
Provide wound management for acute and chronic wounds
Administer routine injections as prescribed.
Support and advise in relation to contraception and sexual health
Provide anticoagulation monitoring.
Deliver travel health care including NHS vaccinations and advice.
Undertake ear assessment and irrigation.
Assess and care for patients presenting with minor injuries and ailments, referring to appropriate clinician as required.
Assist with minor surgery and IUCD insertion.
Undertake home visits to housebound patients requiring immunisations or management of a long-term condition.
Select, carry out and interpret results of specific tests.
Maintain accurate and contemporaneous patient records, both written and electronic e.g. Systmone.
Ensure all statutory regulations e.g. infection control, health and safety, COSHH are implemented and adhered to.
Support the clinical team with all safeguarding matters, in accordance with local and national policies.
Communicate effectively with colleagues, patients and their relatives with are awareness of data protection and information governance.
Mentor and support other staff and health professionals in developing and maintaining clinical skills, including medical students.
Undertake all personal training required to fulfil the current job description and all future requirements relevant to the role.
Participate in annual appraisal, peer review and revalidation as required by practice policy and NMC code of conduct.
Collaborate with the practice team to ensure the practice is compliant with CQC standards.
