Patient Pathway Coordinator

We are seeking an enthusiastic Patient Pathway Coordinator (PPC) to join the established Cancer Services administrative team. This busy role provides support to oncology clinicians. Successful applicants should be team players with excellent organisational and communication skills, and flexible to deliver a patient‑focused, high‑quality, efficient administrative service.

Main duties of the job
To be the point of contact for all patient issues relating to their pathway of care, both for inpatients and outpatients.
To work as part of a multidisciplinary team for the Cancer and Haematology (CSU), supporting the business and operational management of the CSU.
To provide expertise in the proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed smoothly.
To ensure high levels of patient and clinician satisfaction by being an accessible, customer‑focused and knowledgeable point of contact.
To provide a consistent approach across the Trust, Standard Operating Procedures (SOPs) will describe how the functions of this role will be carried out along with the timescales to which they are to be adhered.
Provide comprehensive secretarial/PA support to consultants and clinical teams within the CSU.
Job responsibilities
Main duties and responsibilities
Use Trust IT systems and CSU databases to ensure relevant and appropriate patient information is entered and captured.
Undertake general typing duties for clinic letters, specialist and/or confidential reports (e.g. DVLA, Registration of blind patients, inquests, police reports).
Ensure that referral management is dealt with in accordance with Patient Access Policy (PAP) and Standard Operating Procedures (SOPs).
Ensure that any diagnostics and investigations that are required as part of the patient pathway are requested according to SOP and that the results are available for consultations, including those carried out at other Trusts.
Pathway tracking
To understand cancer and 18‑week referral to treatment (RTT) rules and use them to manage all patient journeys.
Undertake validation of the RTT Patient Tracking List. Investigate and take the appropriate action where pathways are incomplete to ensure that patients are receiving timely treatment and Trust information is robust.
Communications / Customer
Demonstrate high levels of customer care and act as an ambassador for customer care within the Trust.
Act as the communication hub for the clinical team, other PPCs, primary care, patients and their relatives, as well as internal and external organisations.
Ensure that Trust agreed standard of service is delivered to patients requiring elective admission to the speciality.
Ensure details of patients to be admitted are entered onto CRS.

Person Specification
Qualifications and knowledge
* Good general education including English at GCSE (Grade C or above) or equivalent.
* Good standard of spoken English.
* Strong IT skills Word/Excel to NVQ level 3 or equivalent.
* Typing qualification or equivalent experience.
* ECDL qualification or equivalent.
* First line management qualification or relevant supervisory experience.
Experience
* Relevant secretarial/administration experience working within a clinical environment.
* Experience of using IT/patient data systems.
* Ability to accurately input and retrieve information on a computerised system while adhering to complex local and national policies and guidelines.
* Experience of handling inbound and outbound telephone calls of a potentially sensitive nature.
* Demonstrate the ability to delegate tasks and supervise outputs.
* Worked within a hospital environment.
Skills
* Effective verbal and written communication skills / good telephone manner.
* Experience working in a team and developing effective working relationships with staff at all levels.
* Evidence of working under pressure to deadlines and dealing with difficult situations (e.g. patients and public).
