This exciting opportunity has arisen to work specifically in the Older Adult inpatient wards, supporting reduced length of stay and improving the patient journey through our services.

As a registered practitioner, you are personally accountable for your professional practice and have a legal and professional responsibility to service users, colleagues, employers, and yourself.

We are looking for someone who can work autonomously and liaise with external partners to improve patient outcomes.

It is Trust policy that for posts that are for a time-limited period, applicants currently employed by an NHS organization will only be appointed on a secondment basis, with approval from their line manager before applying. Applicants not employed by an NHS organization will receive a fixed-term contract.


Main duties of the job

You will work closely with the Matron and band 4 flow coordinators to ensure safe and timely discharge from Older Adult inpatient wards.

You will provide professional oversight to the band 4 flow coordinators within inpatient settings.

You will represent the Older Adult wards at flow pathway meetings and liaise with system partners to achieve timely discharge from all 4 Older Adult inpatient wards.

You will demonstrate a professional image of nursing to promote a positive image of the Trust for staff, service users, carers, and the community.

Within your role, you will support the Matron by:

* Providing strong clinical leadership, ensuring evidence-based practice is delivered to improve the quality and responsiveness of discharge processes.
* Monitoring and ensuring the quality of Nurse placement assessments and providing education as required.
* Supporting junior colleagues involved in the discharge process.
* Supervising and promoting staff development for the band 4 flow coordinators.

You will have a broad understanding of the Derbyshire pathway discharge process and will form links with wider system partners.

The post holder will receive support from the Matron, HoN, Clinical leads, and ward managers, reporting to the Matron.

Person Specification


Experience

* Recent, relevant post-qualification experience as a Registered Mental Health Nurse
* Community experience
* Experience working with older adults
* Experience with people with Dementia
* Evidence of up-to-date Personal Development Plan/Professional Portfolio


Qualifications

* Supervision trained
* Leadership skills such as motivation, facilitation, mentorship, etc.


Skills

* Knowledge of MHA/Capacity Act/Legal frameworks


Additional Requirements

* Willingness and ability to commute between sites as required
