Overview
We are inviting applications for a highly motivated and dynamic Health Care Assistant to join North Tees & Hartlepool Community Nursing Service. This service will provide care to patients within a range of settings including patients own homes and residential care homes.

Responsibilities
* Provide a whole person approach to care and management for patients within the community setting, working autonomously.
* Liaise with a variety of health and social care providers and other agencies to ensure provision of high quality care management to patients and their carers.
* Care will be provided to patients in their own homes and in residential care homes.
* Work across professional boundaries with excellent communication skills.
Qualifications
* NVQ level 3 in health and social care.
* Excellent communication skills; confident working across professional boundaries.
* Flexible, adaptable and a good team player with the ability to work independently within a community setting.
Working patterns and additional notes
* Be prepared to work all shift patterns within the locality including early and late shifts, weekends and bank holidays.
* There may also be a need to provide cover for the out of hours District Nursing team.
