Siam Surgery has an opportunity for two care coordinators to join our team on a part-time or full-time basis, for a minimum of 22.5 hours per week. The post-holder will provide extra time, capacity and expertise to our practice team to help us to improve the care provided to our patients. The main duties of the role include: Working with GPs and other professionals within the practice to identify and manage a caseload of patients, particularly vulnerable patient groups with long-term conditions that may require extra support. Directing patients to locally available support and services. Supporting the coordination of all key activity, including access to services, advice and information, as well as ensuring that health and care planning is timely, efficient, and patient-centered. Helping patients prepare for, or follow-up after, clinical conversations and hospital discharges. Supporting digital initiatives. Promoting health education. Using clinical systems on a daily basis, with confidence to run searches and keep care records up-to-date. Please see the attached job description for further information. About the Candidate The ideal applicant will have the following skills, experience and qualifications: Experience of working within healthcare or the community sector. Diploma Level 2 in Health and Social Care, or an equivalent qualification (desirable not essential). Familiar with local resources and services, including how to access them. Motivated by helping people, with care and empathy. Strong listening and communication skills. Strong IT skills. Good attention to detail. Able to work well within a team.
