Overview
Assistant Patient Pathway Coordinator (CAT 9) - Fixed Term 12 Months (maternity leave cover). Based in Reading, England, United Kingdom, within the Royal Berkshire NHS Foundation Trust.
Responsibilities
* Process and log all referrals, including paper and ERS, in line with Trust Access & Administration Process Policy and specialty SOPs.
* Schedule new and follow-up appointments, ensuring capacity is proactively and efficiently used. Book patients into the right clinic to ensure they are seen by the most appropriate clinician first time and for subsequent follow-ups.
* Ensure diagnostics and investigations required as part of the patient pathway are requested and results are available for consultations, including those carried out at other Trusts.
* Coordinate appointments and procedures at other hospitals and organisations where the pathway requires input from these.
* Complete clinic cancellation forms and process in accordance with SOPs.
* Reschedule outpatient clinics as requested, in line with waiting time targets.
* Use the Trust’s medical transcription system and ensure letters are processed in accordance with SOPs.
* Provide cover for Pain reception when required.
