Job Title: Social Worker - Generic Location: Darlington Pay Rate: £26 per hour Liquid Personnel is recruiting an Adult Social Worker to join its client’s Generic team, based in Darlington. What will your responsibilities be? Facilitate safe and timely hospital discharges using intermediate care or long‑term support services, ensuring a smooth “home first” transition. Complete person‑centred, strengths‑based assessments and ongoing reviews to support recovery and determine longer‑term care needs. Work both independently and within the RIACT multidisciplinary team while maintaining high‑quality case recording and applying the Care Act 2014 and discharge procedures.
