Join Our Pioneering Population Health Team as a Senior Nurse/AHP
We are seeking a dynamic Population Health Management (PHM) Senior Nurse/AHP to lead an innovative health improvement initiative in Oldham East. You will work across the PCN to identify, engage, and support adults aged 50-64 who are at risk of frailty, collaborating with a dedicated care coordinator to promote proactive, preventative care and maintain health, manage chronic conditions, and prevent deterioration.
Responsibilities
* Drive early intervention strategies to improve health outcomes
* Provide clinical expertise directly to patients with personalised, compassionate care
* Work within an integrated neighbourhood team alongside health and social care professionals, the NCA, and Oldham Council
* Contribute to a pioneering health program that places prevention at the heart of care

Registration
* Registered Nurse with current NMC Pin (Adult)
Qualifications
* Degree/Diploma in nursing studies (or equivalent nursing qualification and experience)
* Evidence of continued professional development
* Evidence of involvement in teaching
Desirable
* Prescribing qualification or willingness to undertake
Knowledge, Skills and Experience
* Evidence-based clinical knowledge
* Competent IT skills
* Clear and effective communication verbally and in writing
* Team collaboration and diplomacy
* Ability to coordinate workload and supervise team members for high-quality, safe patient care
